原发性干燥综合征合并肺间质病变患者血清KL-6水平分析

摘　　要：目的探讨原发性干燥综合征(primary Sjogren’s syndrome,pSS)伴肺间质病变(interstitial lung disease,ILD)患者血清涎液化糖链抗原(Krebs von den Lungen-6,KL-6)水平及其临床意义。方法选择2017年1~12月北京市海淀医院明确诊断为pSS的患者80例,根据肺高分辨率CT(high resolution CT,HRCT)检查,分为pSS不合并ILD组及pSS合并ILD组,每组40例。收集两组患者的一般资料、实验室数据,检测血清KL-6,对各项指标进行比较及相关性分析。对pSS合并ILD组的患者治疗1年,对比治疗前后血清KL-6水平及相关指标的变化。结果pSS合并ILD组血清KL-6浓度[614.00(380.75,921.00)U/ml]明显高于pSS不合并ILD组[213.50(128.25,249.75)U/ml],差异有统计学意义(U=-6.428,P<0.001)。两组均未发现KL-6与年龄、ESR、CRP、IgG的相关性。pSS合并ILD组治疗1年后,血清KL-6浓度[388.00(331.00,529.50)U/ml]较治疗前[687.00(379.00,1098.00)U/ml]降低,差异有统计学意义(U=-2.899,P=0.004)。结论血清KL-6水平在pSS合并ILD患者的诊断中起辅助作用,可评估病情的活动性及监测治疗效果。Objective To investigate the serum Krebs von den Lungen-6(KL-6)level and its clinical significance in primary Sj?gren’s syndrome(pSS)complicated with interstitial lung disease(ILD).Methods The patients diagnosed with pSS from January to December 2017 were divided into two groups(pSS without ILD group and pSS with ILD group)according to lung HRCT examination,40 patients were in each group.General data and laboratory data were collected,serum KL-6 was detected,and various indicators were compared and analyzed.The serum KL-6 and related indicators of the p SS with ILD group were compared before and after treatment for one year.Results The level of serum KL-6 in pSS with ILD group was significantly higher than that in pSS without ILD group[614.00(380.75,921.00)U/ml vs.213.50(128.25,249.75)U/ml,U=-6.428,P<0.001].No correlations between KL-6 and age,erythrocyte sedimentation rate,CRP,IgG were found in pSS with ILD and p SS without ILD group.One year after treatment,the level of serum KL-6 in p SS with ILD group was lower than that before treatment[388.00(331.00,529.50)U/ml vs.687.00(379.00,1098.00)U/ml,U=-2.899,P=0.004].Conclusions Serum KL-6 may play an assistant role in the diagnosis of pSS complicated with ILD,and can evaluate the activity of the disease and monitor the therapeutic effect.
