舒芬太尼复合盐酸戊乙奎醚用于老年无痛胃镜术的临床研究

摘　　要：目的探究舒芬太尼复合盐酸戊乙奎醚用于无痛胃镜术的效果。方法择期行胃镜术的老年患者60例,年龄65~75岁,体质指数(BMI) 18~25 kg/m^2,美国麻醉医师协会(ASA)分级Ⅰ或Ⅱ级。采用随机数字表法,将其分为研究组(30例)和对照组(30例)。在麻醉开始前10 min研究组给予盐酸戊乙奎醚0.01 mg/kg,对照组给予等容量的生理盐水,之后两组患者均给予舒芬太尼0.15μg/kg和丙泊酚1 mg/kg进行麻醉,并根据镇静深度情况适当追加丙泊酚,待警觉/镇静评分(OAA/S)等于1分时开始手术。记录围术期发生呼吸抑制、呛咳、皱眉、肢体活动及术后发生恶心、呕吐、头晕、瘙痒、嗜睡等情况,以及对镇痛的满意率。结果研究组有7例,对照组有15例患者出现了不同程度的呛咳,两组比较差异有统计学意义(P <0.05);研究组相比对照组心动过缓发生率显著降低,差异有统计学意义(P <0.05)。结论在老年患者无痛胃镜术中预注盐酸戊乙奎醚能显著降低舒芬太尼导致的呛咳反应,而且并不增加心动过速的发生率。Objective To evaluate the effect of sufentanil combined with penehyclidine hydrochloride in older patients undergoing painless gastroscopy.Methods 60 patients(age:65~75 years,BMI:18~25 kg/m^2,ASA:I-Ⅱ undergoing painless gastroscopy were randomly divided into research group(30 cases) and control group(30 cases).The research group was given penehyclidine hydrochloride(0.01 mg/kg) and the control group was given saline at 10 min before anaesthesia.All patients were anesthesized with sufentanil 0.1 μg/kg and propofol 1 mg/kg.The surgical operation started when OAA/S score reached one.The respiratory depression,cough,frown,body movement during operation,nausea and vomiting,dizziness,itching,drowsiness after operation,and satisfaction with analgesia were recorded.Results 7 patients in research group and 15 patients in control group had a cough.There was significant difference between the two groups(P <0.05).The incidence of bradycardia in research group was lower than that in the control group(P <0.05).Conclusion Pre-administration of penehyclidine hydrochloride in older patients undergoing painless gastroscopy can significantly reduce the cough reaction caused by sufentanil,and does not increase the incidence of tachycardia.
