补肾排卵汤配合电针治疗未破裂卵泡黄素化综合征型不孕症临床观察

摘　　要：目的观察自拟补肾排卵汤配合电针对黄素化未破裂卵泡综合征(LUFS)促排卵率以及妊娠率的影响。方法将患者按照随机数字表法分为治疗组和对照组,各30例。治疗组从月经第5天开始口服补肾排卵汤配合电针治疗,从月经第10天开始2组均彩超监测卵泡,待卵泡达到18~23 mm时,2组均注射绒促性素(HCG)诱发排卵。72 h后观察卵泡排出例数及卵泡膜血流的阻力指数(RI)和搏动指数(PI)。卵泡排出后,2组均予黄体酮胶丸(200 mg/日)口服。14 d后,观察患者的妊娠情况。连续治疗3个周期。结果治疗组治疗3个疗程后,排卵率及妊娠率明显高于对照组,有明显统计学意义(P<0.05);治疗后卵泡膜血流的RI和PI明显低于治疗前(P<0.05);治疗组有效率明显优于对照组,2组比较差异有统计学意义(P<0.05)。结论补肾排卵汤配合电针不仅可改善卵巢局部的血流参数,增加卵巢血运,而且也可促进卵巢激素的正常分泌,进而促进卵泡的排出,提高妊娠率。Objective To observe the effect of self-made Bushen Pailuan decoction combined with electro-acupuncture on the ovulation promoting rate and pregnancy rate of in luteinized unruptured follicular syndrome(LUFS). Methods Sixty infertile patients with unruptured follicular luteinization syndrome were divided into treatment group( n=30) and control group(n=30) according to random numerical table. From the fifth day of menstruation in the treatment group, self-made Bushen Pailuan decoction combined with electro-acupuncture treatment was applied. The two groups of patients from the 10 th day of menstruation began to monitor follicles by color Doppler ultrasound, when the follicles reached between 18-23 mm. Both groups were injected with(HCG) intramuscularly to induce ovulation. 72 hours later, the number of follicular efflux and the resistance index(RI) and pulsatile index(PI) of follicular efflux and follicular blood flow were observed in both groups. After follicular efflux, the patients in both groups were given progesterone gel pill(200 mg/-day) orally. 14 days later, the pregnancy of the patients was observed.Continuous treatment for three cycles. Results After three treatment courses, the ovulation rate and pregnancy rate was significantly higher than that of the control group, which was statistically significant(P <0.05). In the treatment group, the RI and PI of the flow of the follicle-membrane after treatment were significantly lower than that of the pre-treatment. The effective rate of the treatment group higher than that of the control group, and the difference between the two groups was statistically significant( P<0.05). Conclusion Bushen Pailuan decoction combined with electro-acupuncture can not only improve the blood flow parameters of ovary, increase the blood flow of ovary, but also promote the normal secretion of ovarian hormone, then promote follicular efflux and increase pregnancy rate.
