摘  要:目的研究初诊原发性高血压患者血清尿酸与血压水平及左室肥厚/重构的相关性。方法收集于2016年12月~2019年3月在南京医科大学第二附属医院心内科门诊及住院的初诊未服药的原发性高血压患者122例(高血压组),健康对照58例。同时应用超声心动图结果计算得左室重量指数(LVMI)及相对室壁厚度(RWT),根据2018ESH/ESC高血压管理指南推荐左室肥厚/左室重构的几何构型分类,分为左室肥厚/重构组102例,非左室肥厚/重塑组64例。结果原发性高血压组血清尿酸(349.33±109.75)mmol/L,与健康对照组(279.71±60.58)mmol/L比较,差异有统计学意义(P<0.05);左室肥厚/重构组血清尿酸水平(346.40±113.8)mmol/L,与非左室肥厚/重构组(270.07±78.69)mmol/L比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析发现,尿酸是初诊原发性高血压的危险因素(P<0.05),而不是左室肥厚/重构的危险因素(P>0.05)。结论尿酸与原发性高血压患者血压水平及左室肥厚/重构密切相关。尿酸是原发性高血压发病的独立危险因素,而不是左室肥厚/重构的独立危险因素。Objective To study the relationship between serum uric acid and blood pressure and left ventricular hypertrophy/remodeling in patients with primary hypertension.Methods 122 patients with primary hypertension who were treated in the cardiology inpatient department and hospitalized in Second Affiliated Hospital of Nanjing Medical University from December 2016 to March 2019 and did not take medicine were selected as hypertension group,and 58 healthy people were enrolled in control group.At the same time,the echocardiography was applied to calculate left ventricular mass index(LVMI)and relative wall thickness(RWT),and according to the geometric classification of left ventricular hypertrophy/left ventricular remodeling recommended by the 2018 ESH/ESC hypertension management guidelines,102 cases were divided into left ventricular hypertrophy/remodeling group and 64 cases into non left ventricular hypertrophy/remodeling group.Results There was a significant difference in serum uric acid between the hypertension group(349.33±109.75)mmol/L and the control group(279.71±60.58)mmol/L(P<0.05);There was a significant difference in serum uric acid between the left ventricular hypertrophy/reconstruction group(346.40±113.8)mmol/L and the non-left ventricular hypertrophy/reconstruction group(270.07±78.69)mmol/L(P<0.05).Multivariate Logistic regression analysis showed that uric acid was the risk factor of primary hypertension(P<0.05),not the risk factor of left ventricular hypertrophy/remodeling(P>0.05).Conclusion Uric acid is closely related to the level of blood pressure and left ventricular hypertrophy/remodeling in patients with primary hypertension,and is an independent risk factor for the onset of primary hypertension,rather than an independent risk factor for left ventricular hypertrophy/remodeling.
