摘 要:目的探讨醒脑静注射液对脓毒症大鼠急性肾损伤的改善作用及可能机制。方法(1)将90只大鼠随机分为模型组、处理组、对照组各30只。模型组、处理组采用腹腔注射脂多糖法建立脓毒症急性肾损伤大鼠模型,对照组大鼠腹腔注射等体积生理盐水。造模成功后,处理组大鼠腹腔注射醒脑静注射液,对照组与模型组大鼠腹腔注射等体积生理盐水。观察3组大鼠一般状况;干预后6、12、24 h,检测各组大鼠血清肌酐、尿素氮水平,以及肾组织中肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)、B细胞淋巴瘤-2(Bcl-2)相关X蛋白(Bax)、Bcl-2水平。(2)取肾小管上皮细胞,采用脂多糖法制备急性肾损伤模型细胞,将模型细胞分为模型组、处理组、核因子κB(NF-κB)激活组。处理组细胞加入醒脑静注射液,NF-κB激活组细胞先后予醒脑静注射液、NF-κB激活剂处理,模型组细胞不做处理。培养48h后,检测各组细胞的NF-κB蛋白、TNF-α、IL-6水平及细胞凋亡率。结果(1)模型组及处理组大鼠均出现脓毒症症状,但处理组大鼠的症状减轻。干预后6、12、24h,对照组、处理组、模型组大鼠血清肌酐及尿素氮水平、肾组织TNF-α、IL-6水平及Bax蛋白表达水平均依次升高,而肾组织Bcl-2蛋白表达水平则依次降低(均P<0.05)。(2)NF-κB激活组、处理组细胞的NF-κB、TNF-α、IL-6水平及细胞凋亡率均较模型组降低,且处理组细胞的NF-κB、TNF-α、IL-6水平低于NF-κB激活组(均P<0.05)。结论醒脑静注射液可减轻脓毒症大鼠的症状及急性肾功能损伤,这可与其调控NF-κB活性,从而抑制肾小管上皮细胞凋亡及炎性因子释放有关。Objective To explore the role and potential mechanism of Xingnaojing Injection for improvement of acute kidney injury in septic rats.Methods(1)Ninety rats were randomly divided into model group,treatment group and control group,with 30 rats in each group.Rat model of sepsis plus acute kidney injury was developed in the model and treatment groups by intraperitoneal injection of lipopolysaccharide,whereas rats in the control group were intraperitoneally injected with equivalent volume of normal saline.After successful modeling,rats in the treatment group received Xingnaojing Injection intraperitoneally,while rats in the control and model groups were intraperitoneally injected with equivalent volume of normal saline.The general condition of rats was observed in the three groups;at 6,12,24 hours after intervention,the levels of serum creatinine,blood urea nitrogen(BUN),as well as the levels of tumor necrosis factorα(TNF-α),interleukin 6(IL-6),B-cell lymphoma-2 associated X protein(Bax),and B-cell lymphoma-2(Bcl-2)in renal tissues were detected in each group.(2)Renal tubular epithelial cells were collected to prepare the model cells of acute kidney injury by lipopolysaccharide,and the model cells were divided into model group,treatment group and nuclear factorκB(NF-κB)activation group.The cells in the treatment group were added with Xingnaojing Injection,the cells in the NF-κB activation group were treated with Xingnaojing Injection and NF-κB activators successively,and the cells in the model group did not receive any treatment.Forty-eight hours after culture,the levels of NF-κB protein,TNF-αand IL-6 as well as the rate of cell apoptosis were detected in the cells of each group.Results(1)Rats in the model and treatment groups all exhibited symptoms of sepsis,while rats in the treatment group achieved symptom remission.At 6,12 and 24 hours after intervention,the levels of serum creatinine and BUN as well as the levels of TNF-α,IL-6 and Bax protein expression in renal tissues increased in the control group,the
