低位摆放呼吸机管路预防呼吸机相关性肺炎的临床价值

摘　　要：目的探讨低位摆放呼吸机管路对预防呼吸机相关性肺炎的临床价值。方法2016年1月-2017年9月本院收治的急性呼吸衰竭患者90例,按照随机数字法分为两组,每组均为45例,对照组呼吸机管路实施常规摆放,以"T"型支架固定,观察组实施本研究低位摆放。结果相较于对照组,观察组的救治成功率明显提高(P<0.05),观察组的冷凝水倒流率及呼吸机相关性肺炎率明显降低(P<0.05),使用呼吸机治疗时间较短(P<0.05),入住ICU时间较短(P<0.05),干预后1 d、干预后7 d,TNF-α水平均较同期对照组低(P<0.05)。结论低位摆放呼吸机管路在预防呼吸机相关肺炎中具有重要的应用价值,可有效增强呼吸机供氧患者的总抗氧化能力,减小炎症影响,提高救治成功率。Objective To investigate the clinical value of placing ventilator ducts at low position to prevent ventilator-associated pneumonia.Methods 90 cases from January 2016 to September 2017,90 patients with acute respiratory failure admitted to our hospital were divided into two groups according to random number method,with 45 patients in each group.The control group ventilator pipeline was routinely placed in"T"scaffolds were fixed in the observation group.The observation group was placed in the low position.Results The incidence of ventilator-associated pneumonia,the reflux rate of condensate water and the successful treatment rate in the observation group were significantly lower than those in the control group(P<0.05).The treatment time of the ventilator in the observation group was significantly shorter than that in the control group(P<0.05).The ICU time was significantly shorter in the observation group than in the control group(P<0.05),1 day after the intervention and 7 days after the intervention.The levels of TNF-αwere significantly lower than those in the control group(P<0.05).Conclusion For those who use ventilator,the ventilator pipeline is placed at a low position to reduce the body’s inflammatory response and increase the total antioxidant capacity,thus significantly improving the improving the success rate of treatment.
