摘 要:目的探究侧卧位颈部不同屈伸状态下各肌肉表面肌电特点及意义。方法以2018年10月在深圳市宝安区人民医院进行健康体检的60例志愿者作为研究对象,在颈部不同的屈伸状态(颈部前屈、中立和后伸)下,对胸锁乳突肌、颈竖脊肌和斜方肌左上部三处的肌肉表面点信号进行测定,比较侧卧位颈部不同屈伸状态下,各部位及左右两侧积分电值(IEMG)和中位频率(MF)的变化差异。结果颈部体位保持为后伸位置时,胸锁乳突肌的积分电值要显著高于前屈位和中立位,差异有统计学意义(P<0.05);颈部体位保持为后伸位置时颈竖脊肌的积分电值显著低于前屈位和中立位,差异有统计学意义(P<0.05)。不同颈部体位下胸锁乳突肌、颈竖脊肌和斜方肌左上部的中位频率值之间差异无统计学意义(P>0.05)。颈部呈后伸位时,胸锁乳突肌左右两侧积分电值均显著高于前屈位和中立位(P<0.05),颈竖脊肌左右两侧颈部后伸位的积分电值显著低于前屈位和中立位(P<0.05),斜方肌左上部在不同体位和左右两侧的积分电值均不存在统计学差异(P>0.05)。侧卧位颈部不同屈伸状态下胸锁乳突肌、颈竖脊肌和斜方肌左上部以及左右侧的中位频率值差异无统计学意义(P>0.05)。结论在侧卧位睡眠时,颈部后伸时颈竖脊肌的紧张程度更低,可以更好的促进睡眠,减少患者睡眠过程中的疲惫感。Objective To explore the characteristics and significance of surface electromyography of muscles in lateral decubitus position with different flexion and extension of neck.Methods The muscle surface point signals of the left upper part of the sternocleidomastoid,the cervical and the trapezius muscle and the left upper part of the trapezius muscle were measured with 60 volunteers who underwent a healthy physical examination in the People′s Hospital of Bao′an District in Shenzhen City in October 2018.The changes of the integral electrical value(IEMG)and median frequency(MF)of the parts and the left and right sides were compared in the different flexion and extension states of the lateral position.Results When the cervical position was maintained as the extension position,the integral electrical value of the sternocleidomastoid muscle was significantly higher than that of the flexion position and the neutral position(P<0.05),and the integral electrical value of the vertical spinal muscle was significantly lower than that of the flexion position and the neutral position(P<0.05).There were no significant differences in the median frequency of the sternocleidomastoid muscle,cervical erector spine muscle and trapezius muscle in the different cervical positions(P>0.05).When the neck was in the extension position,the integral electrical values of the left and right sides of the sternocleidomastoid muscle were significantly higher than those of the flexion position and the neutral position(P<0.05).The integral electrical values of the left and right sides of the vertical spine muscle were significantly lower than those of the flexion position and the neutral position(P<0.05).There was no statistical difference between the left and right sides of the trapezius muscle in the different positions(P>0.05).There were no significant differences in the median frequency of the sternocleidomastoid muscle,vertical cervical spine muscle and trapezius muscle between the left and right sides(P>0.05).Conclusion During sleeping with t
