微小单孔VATS治疗老年原发性气胸及术后血清TNF-α、IL-1、hs-CRP的变化

摘　　要：目的探究微小单孔电视辅助胸腔镜手术(VATS)治疗老年原发性气胸及术后血清学指标的变化情况。方法选取2016年5月-2018年5月,在邢台医学高等专科学校第二附属医院胸外科就诊的80例60岁以上老年原发性气胸拟行手术患者,采用随机数表分为观察组和对照组,每组各40例,两组患者行基础治疗后,对照组患者行传统胸腔镜手术治疗,观察组患者行微小单孔VATS治疗,术后7 d内行相同术后护理,比较两组患者术后疗效、血清炎症指标及不良反应发生情况的差异。结果与对照组比较,观察组患者术后引流管留置时间、术后引流量、术中出血量及住院时间均降低,差异有统计学意义(P<0.05);观察组患者血清肿瘤坏死因子-α(TNF-α)、白介素-1(IL-1)、超敏C反应蛋白(hs-CRP)和不良反应发生率均降低(P<0.01)。结论微小单孔VATS治疗老年原发性气胸与传统治疗方法相比具有更好的手术治疗效果,且安全性更佳。Objective To investigate the changes of serum indexes in the treatment of senile primary pneumothorax with VATS.Methods From May 2016 to may 2018,eighty patients over 60 years old were selected from the Thoracic Surgery Department of the Second Affiliated Hospital of Xingtai Medical College.They were randomly divided into two groups:observation group and control group,40 cases in each group.After basic treatment,the patients in the control group were treated with traditional thoracoscopic surgery,while the patients in the observation group were treated with micro-single-hole VATS.After treatment,the same postoperative nursing was performed within 7 days after operation,and the differences of postoperative efficacy,serum inflammatory index and adverse reactions between the two groups were compared.Results Compared with the control group,the retention time of drainage tube,postoperative drainage volume,intraoperative bleeding volume and hospitalization time in the observation group were all decreased(P<0.05);the serum TNF-α,IL-1,hsCRP and the incidence of adverse reactions in the observation group were all decreased(P<0.01).Conclusion Compared with the traditional treatment,VATS is more effective and safe in the treatment of senile primary pneumothorax.
