摘　　要：目的探讨T1 mapping在髌骨软化症诊断中的应用价值。方法正常对照组15个膝关节和经关节镜证实的病例组38个膝关节行磁共振成像(MRI)扫描,包括常规MRI扫描和T1 mapping扫描。分别测量正常对照组、病例组髌骨软骨的T1值,采用独立样本t检验比较两组差异,并对髌骨软骨的T1值进行ROC曲线分析。结果病例组38个膝关节,21个在常规MRI扫描图像发现髌骨软骨异常,14个在常规MRI扫描图像未发现异常而在T1 mapping图像发现异常。病例组髌骨软骨的T1值明显高于正常对照组,差异有统计学意义(t=-6.726,P<0.01)。T1 mapping评估髌骨软化症的ROC曲线下面积为0.912,当诊断界值取909.15 ms时,约登指数为0.737,敏感度为73.7%,特异度为100%。结论 T1 mapping可用于评估髌骨软骨损伤,为髌骨软化症的早期诊断提供帮助。Objective To explore the value of T1 mapping in the diagnosis of chondromalacia patellae.Methods 15 knee joints in the control group and 38 knee joints confirmed by arthroscopy in the case group were scanned by MRI,including conventional MRI and T1 mapping.The T1 values of patellar cartilage in the groups were respectively measured and analyzed for their ROC curves using T-test of independent samples thus to compare the differences between the two groups.Results Among the 38 knee joints in the case group,patellar cartilage abnormalities were found in 21 cases by conventional MRI scan images,whereas 14 cases showed no abnormalities in routine MRI scan images but abnormalities in T1 mapping images.The T1 value of patellar cartilage in the case group was significantly higher than that in the control group and differed statistically(t=-6.726,P<0.01).The area under the ROC curve of T1 mapping in the evaluation of chondromalacia patellae was 0.912.When the diagnostic threshold was 909.15 ms,the Youden’s index was 0.737,the sensitivity was 73.7% and specificity was 100%.Conclusion T1 mapping can be used for quantitative assessment of patellar cartilage injury and help to diagnose early chondromalacia patellae.
