摘 要:目的探讨低频电刺激治疗肱骨干骨折并发桡神经损伤的效果及对患者生活质量的影响。方法选取肱骨骨折术后并发桡神经损伤患者80例,随机分为治疗组和对照组,对照组予常规药物治疗,治疗组在对照组基础上加低频电刺激治疗。治疗前后评定腕伸肌、指总伸肌肌力,进行肌电图检测,并评估治疗后生活质量(the MOS item short from health survey,SF-36)量表。结果治疗后,治疗组腕伸肌4~5级例数较治疗前明显增多,2组指总伸肌4~5级例数较治疗前明显增多(P<0.05)。治疗后,治疗组腕伸肌、指总伸肌肌力均较对照组明显改善(P<0.05)。治疗后,2组运动神经传导速度(motor nerve conduction velocity,MCV)及波幅值均较治疗前增高(P<0.05)。治疗后,治疗组MCV及波幅值均高于对照组(P<0.05)。治疗后,治疗组社会功能、生理功能、情感职能、精神健康和总体健康评分均高于对照组(P<0.05)。结论低频电刺激治疗可明显提高桡神经损伤患者上肢肌力,改善其运动功能,提高其生活质量。Objective To observe the effectiveness of low frequency electrical stimulation on the treatment of humeral shaft fracture complicated with radial nerve injury and its effect on quality of life.Methods A total of 80 patients with radial nerve injury after humeral fracture were randomly divided into treatment group and control group.The control group was treated with conventional drugs,and the treatment group was treated with low frequency electric stimulation on the basis of the control group.Before and after treatment,the muscle strength of the extensor carpi and extensor digitorum was assessed,and the electromyogram was measured,and the MOS item short from health survey(SF-36)was evaluated.Results After treatment,the number of extensor carpal muscle grade 4-5 in the treatment group was significantly higher than that before treatment,and the number of total extensor muscle grade 4-5 in the two groups was significantly higher than that before treatment(P<0.05).After treatment,the muscle strength of extensor carpal muscle and total extensor finger muscle in the treatment group was much better than that in the control group(P<0.05).After treatment,the amplitude of MCV and wave in the two groups were significantly higher than those before treatment(P<0.05).After treatment,the amplitude of MCV and wave in the treatment group were higher than those in the control group(P<0.05).After treatment,the scores of social functioning,physical functioning,role emotional,mental health and general health in the treatment group were higher than those in the control group(P<0.05).Conclusion Low frequency pulse electrotherapy can effectively improve the muscle strength and motor function of upper limbs in patients with radial nerve injury,and can improve the quality of life of patients.
