消瘿合剂治疗桥本甲状腺炎临床研究

摘　　要：目的:观察消瘿合剂治疗桥本甲状腺炎的临床疗效,并探讨其作用机制。方法:将100例桥本甲状腺炎患者随机分为治疗组和对照组各50例。治疗组每日早晚饭后服用消瘿合剂30 mL,对照组早晚饭后服用硒酵母片100μg,两组均连续治疗12周。比较两组患者治疗前后甲状腺功能、中医症状积分和血清Th1/Th2细胞因子水平,评价疗效。结果:治疗组治疗后血清甲状腺过氧化物酶抗体(TPO-Ab)、甲状腺球蛋白抗体(TgAb)水平、中医症状积分均显著降低,与本组治疗前及对照组治疗后比较,差异有统计学意义(P<0.05)。治疗后,治疗组血清干扰素-γ(IFN-γ)、白细胞介素-2(IL-2)水平显著下降,血清白细胞介素-10(IL-10)水平显著升高,与本组治疗前及对照组治疗后比较,差异均有统计学意义(P<0.05)。治疗组、对照组总有效率分别为79.55%、33.33%,治疗组总有效率高于对照组,差异有统计学意义(P<0.05)。结论:消瘿合剂治疗桥本甲状腺炎能通过纠正患者血清Th1/Th2细胞因子失衡改善临床症状、提高疗效。Objective:To observe the clinical curative efficacy of Xiaoying Mixture(消瘿合剂)in the treatment of Hashimoto’s thyroiditis and explore the related mechanism.Methods:A total of 100 patients with Hashimoto thyroiditis were randomly divided into treatment group and control group,with 50 cases in each group.Patients in the treatment group took 30 m L Xiaoying Mixture everyday in the morning and evening and patients in the control group took 100μg selenium yeast tablet everyday in the morning and evening.Both the two groups were treated for 12 weeks.The thyroid function,traditional Chinese medicine(TCM)symptom score and serum Th1/Th2 cytokines level of the two groups before and after treatment were compared,and the curative efficacy of the two groups were evaluated.Results:After treatment,the levels of serum thyroid peroxidase antibody(TPO-Ab),thyroglobulin antibody(TgAb)and TCM symptom score of the treatment group were significantly lower than those before treatment(P<0.05),and were also lower than those of the control group after treatment(P<0.05).After treatment,in the treatment group,the levels of serum interferon-γ(IFN-γ)and interleukin-2(IL-2)were significantly lower than those before treatment(P<0.05),and the level of serum interleukin-10(IL-10)was significantly higher than that before treatment(P<0.05).Compared with those of the control group after treatment,the differences were also statistically signifficant(P<0.05).The total effective rate of the treatment group and the control group was79.55%and 33.33%,respectively.The total effective rate of the treatment group was higher than that of the control group and the difference was statistically significant(P<0.05).Conclusion:In the treatment of Hashimoto thyroiditis,Xiaoying Mixture can relieve the clinical symptoms and improve the curative efficacy by correcting the imbalance of Th1/Th2 cytokines.
