摘　　要：目的探讨在外科开展专科临床药师轮转制查房模式的重要性、工作方法和效果。方法专科临床药师在外科进行轮转制药学查房,实时为临床医师提供用药指导,针对临床存在的主要专科用药问题,实施药学干预。结果专科临床药师可以改善外科抗菌药物、抗肿瘤药物、抗凝/止血药物和辅助用药等存在的用药问题,增加医护与药学人员的互动,增强专科临床药师临床干预的主观能动性;外科总用药合理率从干预前80.1%提高到干预后的94%,具有显著成效(P<0.05)。结论专科临床药师在外科轮转制查房可以高效地解决专科用药问题,提高外科临床用药的规范性与合理性。OBJECTIVE To study the significance,working method and effect about specialized clinical pharmacists ward round based on rotation mode.METHODS Specialized clinical pharmacists real-time provided medication guidance for clinicians by surgical ward round based on rotation mode,and carried out pharmaceutical intervention according to problems of clinical medication.RESULTS Clinical pharmacists could correct clinical medication problems of surgery such as antimicrobial,antitumor,anticoagulant/hemostatic and adjuvant drugs.Moreover,clinical pharmacists strengthened communication with medical staff and enhanced subjective initiative of clinical pharmacy work.After rotation ward round,the percentage of medication rationality in surgery increased from 80.1%to 94%,with significant difference(P<0.05).CONCLUSION Specialized clinical pharmacists can effectively solve medication problems and improve standardization and rationality of medication by rotation ward round in surgery.
