摘  要:目的探讨一种根据鼻窦冠状位CT图上钩突水平段的影像学分型方法,以期为鼻窦内镜手术提供指导,减少术中眶壁纸样板损伤相关并发症。方法在鼻窦冠状位高分辨CT图上,以钩突水平段与垂直段结合部为基点,做一条垂直线,以同平面眼眶内壁中点做另一垂直线,以经眼眶内侧壁中点垂线为参考,根据两条垂线的关系将钩突水平段分为线外型,切线型和线内型3种类型。结果151例研究对象(302侧眼眶内壁)根据该分型方法进行分型后,线外型88侧,占29.13%;切线型123侧,占40.73%;线内型91侧,占30.13%,线外型眶壁纸样板距离钩突约(2.44±1.27)mm,线内型距离钩突(2.69±1.18)mm。结论按照此分型方法对钩突水平段进行分型可以起到预判手术难度并在一定程度上起到预防眶壁纸样板损伤的作用。Objective To investigate the imaging classification method based on the horizontal segment of the uncinate process in the coronal CT of the sinus and to provide guidance for endoscopic sinus surgery and reduce the complications associated with postoperative stenosis.Methods On the high-resolution CT image of the sinus coronal position,a vertical line was made with the joint of the horizontal section and the vertical section of the uncinate process and the vertical line of the inner wall of the same plane was made to make another vertical line,which is a reference.Then the horizontal section of the uncinate process is divided into three types according to the relationship of two perpendicular lines,a tangential type and an in-line type.Results A total of 151 subjects(302 sides of the orbital wall)were classified according to the classification method.The outside line type was 88 side,which accounted for 29.13%;tangential type was 123 side,which accounted for 40.73%;line type was 91side,which accounted for 30.13%,the outside line type orbit wallpaper template distance hook protrusion(2.44±1.27)mm,the inside line type distance knuckle(2.69±1.18)mm.Conclusion According to this classification method,the classification of the horizontal segment of the uncinate process can play a role in predicting the difficulty of surgery and prevent the damage of the wallpaper sample.
