聚焦式护理对COPD出院患者生活质量及肺功能的影响

摘　　要：目的探讨聚焦式护理对慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)出院患者生活质量(Quality of life,QOL)以及肺功能的影响。方法选择本院呼吸内科2014年收治的120例COPD患者作为研究对象,采用数字随机法将其分为对照组和观察组两组,每组60例。其中对照组患者进行常规出院指导,观察组患者则进行聚焦式护理,比较两组患者的QOL评分和肺功能指标。结果两组患者出院前的VC、FEV1、FEV1/FVC、PEF等肺功能指标差异均不大,但出院6个月后,观察组患者的肺功能指标显著好于对照组(P<0.05);两组患者出院时CAT评分差异均不大,但出院6个月后,观察组患者的CAT评分显著低于对照组(P<0.05);两组患者出院时MMRC评分差异均不大,但出院6个月后,观察组患者的MMRC评分显著低于对照组([WTBX]P[WTBZ]<0.05);观察组患者出院3个月后、出院6个月后的复发率分别为3.33%、8.33%,显著低于对照组的20.00%、31.67%(P<0.05)。结论聚焦式护理对COPD出院患者的效果更好,既改善了患者的肺功能,还提高了患者的生活质量,降低了疾病复发率,可在临床上广泛推荐应用。Objective To study the effect of focus nursing on quality of life(QOL)and pulmonary function in patients with chronic obstructive pulmonary disease(COPD)discharged from hospital.Methods 120 patients with COPD admitted to our hospital in 2014 were selected as our subjects.They were randomly divided into two groups by random number method,which was control group and observation group,each group had 60 cases.The control group received routine discharge guidance,while the observation group patients underwent focus nursing.QOL scores and pulmonary function indices were compared between two groups.Results The pulmonary function indexes of VC,FEV1,FEV1/FVC and PEF were not significant in both groups.However,after 6 months of discharge,the pulmonary function index of patients in the observation group was significantly better than that of control group(P<0.05).After 6 months of discharge,the CAT score of patients in the observation group was significantly lower than of control group(P<0.05).There was no significant difference in MMRC scores between the two groups,but after 6 months of discharge,the MMRC score in the observation group was significantly lower than that of in the control group(P<0.05).After discharge for 3 months in the observation group,the recurrence rate after 6 months of discharge was 3.33%and 8.33%,which was significantly lower than 20.00%,31.67%of control group(P<0.05).Conclusion The effect of focus nursing on patients with COPD is better,which not only improves the lung function of patients,but also improves the quality of life of patients,reduces disease recurrence rate,so it can be widely used in clinic.
