骨生化相关指标与骨折愈合关系的研究

摘　　要：目的:探讨围手术期钙磷乘积和血清25羟维生素D3[25(OH)D3]与创伤骨折预后的关系。方法:收集2016年1月-2018年12月在本院诊断为四肢长骨骨折96例患者的资料,分别获取患者术前24 h、次日清晨、术后1周及术后1个月生化指标,根据影像学资料分为骨性愈合和延迟愈合,根据术前损伤情况分为闭合性骨折与开放性骨折,比较患者钙磷乘积和血清25(OH)D3的水平。结果:骨性愈合者术后1周、术后1个月钙磷乘积、25(OH)D3均明显高于延迟愈合者(P<0.05);闭合性骨折术后1周、术后1个月钙磷乘积、25(OH)D3均明显高于开放性骨折(P<0.05)。结论:术后1~4周血清钙磷乘积、25(OH)D3不足可能会导致骨折患者发生延迟愈合,钙磷乘积、25(OH)D3的水平和变化幅度在预测骨折预后方面有着积极的意义,且性价比高、方法简便。Objective:To investigate the relationship between calcium-phosphorus product,25(OH)D3 and traumatic fracture in the perioperative period.Method:The data of 96 patients with long bone fracture of limbs from January 2016 to December 2018 were collected.Biochemical indexes were obtained at 24 h before operation,the morning of the next day after operation,1 week after operation and 1 month after operation,imaging data of patients were tracked and the patients were divided into bone healing and delayed healing,according to the position of the patients,they were divided into closed fracture and compound fracture,calcium-phosphorus product and 25(OH)D3 of patients were compared.Result:1 week after operation and 1 month after operation,the in crease of calciumphosphorus product and 25(OH)D3 in bone healing were significantly higher than those of delayed healing(P<0.05),the in crease of calcium-phosphorus product and 25(OH)D3 in closed fracture were higher than those of compound fracture(P<0.05).Conclusion:Deficiency of serum calcium-phosphorus product and 25(OH)D3 at 1-4 weeks after operation may lead to delayed healing of fracture patients.Calcium-phosphorus product,25(OH)D3 level and variation range have positive significance in predicting the prognosis of fracture,and the method is simple and cost-effective.
