不同入路小切口髋关节置换术在老年创伤性股骨颈骨折中的疗效比较

摘　　要：目的:探讨不同入路小切口髋关节置换术在老年创伤性股骨颈骨折中的疗效。方法:选择老年创伤性股骨颈骨折患者60例作为研究对象,按照不同手术入路方式分为对照组和观察组,各30例。对照组采取外侧小切口髋关节置换术进行治疗,观察组采取后路小切口髋关节置换术进行治疗,比较2组患者围术期相关指标、疼痛评分(VAS)、日常生活能力评分,比较2组患者治疗后髋关节Harris功能评分。结果:观察组手术时间(88.45±10.32)min、术中出血量(390.5±81.37)m L、术后引流量(145.55±21.60)m L均优于对照组各手术指标(P<0.05)。治疗前,2组疼痛、日常生活能力评分比较均无统计学意义(P>0.05);治疗后,观察组VAS评分[(2.07±0.13)分]低于对照组[(3.62±0.42)分],日常生活能力[(81.31±12.95)分]高于对照组[(72.29±10.44)分](P<0.05)。2组患者髋关节Harris功能评分差异均无统计学意义(P>0.05)。结论:外侧小切口髋关节置换术与后路小切口髋关节置换术应用在老年创伤性股骨颈骨折中均有疗效,但后路小切口髋关节置换术出血量少,治疗过程中患者疼痛感低,治疗后患者日常生活能力较高。Objective:To explore the effect of different approaches to small incision hip joint replacement in elderly patients with traumatic femoral neck fractures.Methods:A total of 60 elderly patients with traumatic femoral neck fractures treated were selected as subjects.They were divided into control group(n=30)and observation group(n=30)according to different surgical approaches.The control group was treated with lateral small incision hip arthroplasty while the observation group was treated with posterior small incision hip arthroplasty.Perioperative indicators,VAR,and VAS scores were compared between the two groups.The Harris function scores of the two groups were compared after treatment.Results:The operation time(88.45±10.32)min,intraoperative blood loss(390.5±81.37)m L,and postoperative drainage(145.55±21.60)m L in the observation group were better than those in the control group(P<0.05).Before treatment,there was no significant difference in pain and daily life scores between the two groups(P>0.05);after treatment,the VAS score[(2.07±0.13)points]in the observation group was lower than the control group[(3.62±0.42)points]The daily living ability[(81.31±12.95)points]was higher than the control group[(72.29±10.44)points](P<0.05).The Harris function scores of the hip joints of the two groups were not statistically significant(P>0.05).Conclusion:Lateral small incision hip replacement is similar to the effect of posterior small incision hip replacement in elderly traumatic femoral neck fractures,but posterior small incision hip replacement has less bleeding and lower pain during treatment.
