芪参益气滴丸治疗收缩性心力衰竭合并肌酐清除率下降的临床疗效观察

摘　　要：目的探讨芪参益气滴丸对合并肌酐清除率(Ccr)下降的收缩性心力衰竭病人的临床疗效。方法采用随机对照研究方法,选取2016年1月-2017年3月在郑州大学附属洛阳中心医院心血管内科住院治疗的合并Ccr下降的收缩性心力衰竭病人150例,随机分为芪参益气滴丸组和对照组。两组均给予常规药物治疗及低盐、低蛋白饮食、适度运动等指导,芪参益气滴丸组在对照组治疗基础上加用芪参益气滴丸,治疗1年。监测两组治疗前后脑钠肽(BNP)、Ccr、左室射血分数(LVEF)及6 min步行距离等指标,观察两组病人开始治疗后1年内的心血管疾病病死率。结果共入选150例病人,最终完成138例。两组基线资料相似,用药后芪参益气滴丸组BNP、Ccr及LVEF、6 min步行距离较对照组明显改善,差异有统计学意义(P<0.05);芪参益气滴丸组1年内心血管病死率较对照组下降,差异有统计学意义(P<0.05)。结论对收缩性心力衰竭合并Ccr下降的病人,予芪参益气滴丸治疗能够改善心功能,提高LVEF、6 min步行距离,降低BNP水平,且可能对肾功能有保护作用。Objective To evaluate the effect of Qishen Yiqi drop pills(QYDP)in the treatment of systolic heart failure patients with reduced creatinine clearance.Methods One hundred and fifty patients with systolic heart failure were randomly divided into two groups:the control group treated with routine treatment,and QYDP group treated with QYDP in the basis of routine treatment for 1 year.The level of brain natriuretic peptide(BNP),creatinine clearance(Ccr),left ventricular ejection fraction(LVEF),and six-minute walking distance were recorded before and during the treatment.The incidence of cardiovascular death was compared between two groups.Results A total of 150 patients were collected,and 138 patients completed one-year follow-up.There was no difference in the baseline data between two groups.Compared with that in the control group,the levels of BNP,Ccr,LVEF,and 6-minute walking distance were significantly improved in QYDP group after treatment,and the differences were statistically significant.The cardiovascular mortality in QYDP group decreased within one year compared with the control group,and the difference was statistically significant.Conclusion QYDP can not only improve cardiac function and prognosis of systolic heart failure patients with decreased creatinine clearance,but also has potential protective effects on renal function.
