摘　　要：目的探讨Orem自理模式对全膝关节置换术患者自理能力及心理弹性的影响。方法对2017年2月一2019年3月在吉林大学第一医院接受全膝关节置换术的1700例患者的临床资料进行回顾性分析。其中对照组有850例患者,采用传统的护理方式,观察组有850例患者,采用Orem自理模式。记录并比较两组患者在术后各时间点的护理知识知晓率、完全自理率,早期关节活动度及膝关节功能评分,心理弹性量表评分及护理满意率。结果术后第8天观察组患者的护理知识知晓率、完全自理率均高于对照组,差异有统计学意义(P<0.05)o观察组患者在术后1周、3周、6周的关节活动度及膝关节功能评分均高于对照组,差异有统计学意义(P V0.05)。观察组患者在术后2周、4周、8周时的心理弹性量表评分均高于对照组,差异有统计学意义(P<0.05)。观察组患者的总满意率高于对照组,差异有统计学意义(PV0.01)。结怡在全膝关节置换术患者术后对其进行Orem自理模式的干预.能够明显提升患者的自理能力和心理弹性,促进患者膝关节功能恢复,增加患者对于护理质量和工作的满意程度。Objective To explore the effect of Orem self-care model on self-care ability and psychological elasticity of patients undergoing total knee arthroplasty.Methods The clinical data of 1700 patients who underwent total knee arthroplasty in the first Hospital of Jilin University from February 2017 to March 2019 were analyzed retrospectively.Among them,850 patients in the control group were treated with traditional nursing,and 850 patients in the observation group were treated with Orem self-care mode.The awareness rate of nursing knowledge,the rate of complete self-care,the score of early range of motion and knee joint function,the score of psycho-elasticity scale and the rate of nursing satisfaction were recorded and compared between the two groups at each time point after operation. Results On the 8th day after operation,the awareness rate of nursing knowledge and the rate of complete self-care in the observationgroup were significantly higher than those in the control group (P V0.05). The scores of range of motionand knee joint function in the observation group were significantly higher than those in the control group at1 week, 3 weeks and 6 weeks after operation (P VO.05). The scores of psycho-elasticity scale in the observationgroup were significantly higher than those in the control group at 2 weeks, 4 weeks and 8 weeks afteroperation (P <0.05). The total satisfaction rate of patients in the observation group was higher thanthat in the control group, and the difference was statistically significant (P V0.05). Conclusion The interventiono￡ Orem self-care mode after total knee arthroplasty can significantly improve the patients' selfcareability and psychological elasticity and promote the recovery of knee joint function, and increase patients7satisfaction with nursing quality and work.
