摘　　要：目的:探讨早期平均血小板体积/淋巴细胞比值(MPVLR)对急性脑梗死的诊断价值。方法:选取首发并于发病24 h内入院的急性脑梗死患者189例为研究对象,另选取同期体检者126例为对照组。治疗前采用美国国立卫生研究院卒中量表对脑梗死患者病情严重程度进行评分,轻度117例,中度46例,重度26例。检测两组血常规及血脂指标,计算MPVLR。结果:脑梗死组MPV、MPVLR均高于对照组,而LC低于对照组(P<0.05)。轻度脑梗死患者MPVLR低于重度患者,而LC高于重度患者(P<0.008)。MPV、LC及MPVLR诊断急性脑梗死的ROC曲线下面积分别为0.812(0.765~0.860)、0.792(0.741~0.844)、0.851(0.807~0.895),MPVLR的曲线下面积较大。临界值取5.98时,MPVLR诊断急性脑梗死的灵敏度和特异度分别为74.60%和81.75%。结论:早期MPVLR对急性脑梗死的诊断有一定的价值。Aim:To investigate the value of early mean platelet volume-to-lymphocyte ratio(MPVLR)for diagnosis of acute cerebral infarction(ACI).Methods:A total of 189 patients with ACI who were first diagnosed and admitted within 24 hours were selected as the study objects,and 126 individuals accepted physical examination in the same period were selected as the control group.Before treatment,the stroke scale of the National Institutes of Health was used to evaluate the severity of cerebral infarction,and there were 117 mild cases,46 moderate cases and 26 severe cases.The blood routine and blood lipid indexes were measured,and MPVLR was calculated.Results:The mean platelet volume(MPV)and MPVLR of ACI group were significantly higher than those of the control group(P<0.05),while the lymphocyte count(LC)was lower than those of the control group(P<0.05).MPVLR level of mild cerebral infarction patients was lower than those of the severe cerebral infarction patients,while LC was higher(P<0.08).The area under the ROC curve of MPV,LC,MPVLR for ACI diagnosis were 0.812(0.765-0.860),0.792(0.741-0.844),0.851(0.807-0.895),respectively.When the cut-off value of MPVLR was 5.98,the sensitivity for diagnosis of ACI was 74.60%,and the specificity was 81.75%.Conclusion:Early MPVLR has certain clinical significance for ACI diagnosis.
