摘  要:目的:了解136所三级公立医院医生的心理健康自评现状及影响因素。方法:基于“全国改善医疗服务行动计划第三方评估”(第三轮)对136所三级公立医院的调查结果,使用SPSS 21.0软件,采用卡方检验、二分类多因素Logistic回归分析医生心理健康自评的影响因素。结果:医生的心理健康自评不容乐观(医生选择“好”的比例为34.9%)。多因素分析显示,感知压力越大、值夜班频率越高、近一年曾经历过医患纠纷、近一年曾经历过来自患方的暴力是危险因素(P<0.05),认同“睡眠时间充足”“同事间配合与信任度高”“职业得到社会认同与尊重”“本院收入分配制度起到激励作用”和“本院晋升制度合理”是医生心理健康自评的保护因素(P<0.05)。结论:合理减轻医生工作负荷、疏导医生感知压力,构建和谐医患关系,优化医院收入分配制度和晋升制度是改善医生心理健康的重要举措。Objective: To understand the current status and influencing factors of self-assessment of mental health among doctors in 136 tertiary public hospitals. Methods: Based on the survey results of 136 third-level public hospitals based on the "Third-Party Evaluation of the National Action Plan for Improving Medical Services" (third round), using SPSS 21.0 software, Chi-square test, two-class multi-factor Logistic regression analysis of doctors' mental health Influencing factors of self-evaluation. Results: The self-assessment of doctors' mental health was not optimistic (34.9% of doctors chose "good"). Multivariate analysis showed that the greater the perceived stress, the higher the frequency of night shifts, the past one year experience of doctor-patient disputes, and the past year experience of violence from the affected parties were risk factors (P<0.05). Adequate "" cooperation among colleagues and high trust "," professional social recognition and respect", "the hospital's income distribution system plays an motivating role", and "the hospital's promotion system is reasonable" are protective factors for doctors' mental health self-assessment (P< 0.05). Conclusion: Reasonably reducing the workload of doctors, channeling doctors' perception of pressure, building a harmonious doctor-patient relationship, and optimizing hospital income distribution system and promotion system are important measures to improve the mental health of doctors.
