MDT在老年骨折患者围手术期管理的安全保障

摘　　要：目的:探讨临床多学科协作团队(multi disciplinary team,MDT)在老年骨折患者围手术期管理中的安全保障作用。方法:运用MDT模式,由骨科、老年内科、麻醉科、重症监护室、康复科、营养科、精神保健科共同组成团队,以老年骨折患者为中心,制定规范化的围手术期管理方案、流程和实施细则,对患者实施术前、术中、术后全程多学科协同管理。结果:老年骨折患者围手术期管理MDT建立健全工作机制,自2018年3月开展工作以来,老年髋部骨折手术患者的平均住院日同比缩短1.6天/人次,患者满意度提升8.15%,MDT作用效果显著。结论:以患者为中心的MDT共同制定围手术期管理方案和目标,有效促进了方案和流程的规范化,提升了老年骨折患者在围手术期的安全性,MDT机制在老年骨折患者围手术期管理中起到积极作用。Objectives: To identify the role of MDT( multiple discipline team) in peri-operational period of fracture of elder patients. Methods: The MDT group included orthopedists, geriatricians, anesthetists, ICU specialists, rehabilitation specialist, nutritionist and psychiatrists. Patient-oriented peri-operational management included surgery plan, procedures and practice guidelines. Patient was integrated managed in pre-operation, operation and postoperation. Results: Since implementing MDT in March 2018, the average length of stay has been reduced 1.6 days and patient satisfaction rate increased 8.15%. MDT results in active outcome of patients’ prognosis. Conclusions: Patient centered MDT practices the active role in the patients’ management of peri-operational period, overcomes the defects and flaws of the sole practice of the orthopedists, and improves patients’ safety and the quality of care in peri-operation period.
