复方丹参滴丸联合氯吡格雷治疗慢性心力衰竭合并室性心律失常的疗效

摘　　要：目的:研究复方丹参滴丸联合氯吡格雷治疗慢性心力衰竭(CHF)合并室性心律失常(VA)的疗效。方法:2017年3月至2018年3月在我院就诊的CHF+VA患者92例,患者被随机均分为常规治疗组和联合治疗组(在常规治疗组基础上加用复方丹参滴丸和氯吡格雷),两组均治疗8周。观察比较两组治疗前后左室舒张末期内径(LVEDd)、左室收缩末期内径(LVESd)、血小板聚集率(PAR)、血清C反应蛋白(CRP)水平、校正QT间期离散度(QTcd)及治疗总有效率。结果:与治疗前比较,治疗后两组LVEDd、LVESd、PAR、血清CRP水平、QTcd均显著降低(P均=0.001)。与常规治疗组比较,联合治疗组治疗后LVEDd [(46.32±3.52)mm比(41.21±4.11)mm]、LVESd [(40.68±4.42)mm比(35.18±4.16)mm]、PAR [(7.56±1.25)%比(4.53±1.02)%]、血清CRP水平[(7.45±1.75)ng/L比(5.11±1.59)ng/L]、QTcd[(55.51±8.16)ms比(46.13±7.75)ms]降低更显著(P均=0.001)。联合治疗组总有效率显著高于常规治疗组(97.83%比82.61%,P=0.035)。结论:复方丹参滴丸联合氯吡格雷治疗CHF+VA疗效显著,可显著改善患者心功能,减轻炎症反应,抑制血小板聚集,改善患者VA症状。Objective:To study therapeutic effect of compound Salvia dripping pill combined clopidogrel on chronic heart failure(CHF)complicated ventricular arrhythmia(VA).Methods:A total of 92 CHF+VA patients in our hospital from Mar 2017 to Mar 2018 were randomly and equally divided into routine treatment group and combined treatment group(received compound Salvia dripping pill combined clopidogrel based on routine treatment group).Both groups were treated for eight weeks.Left ventricular end-diastolic dimension(LVEDd),left ventricular endsystolic dimension(LVESd),platelet aggregation ratio(PAR),serum level of C reactive protein(CRP)and corrected QT interval dispersion(QTcd)before and after treatment and total effective rate were observed and compared between two groups.Results:Compared with before treatment,there were significant reductions in LVEDd,LVESd,PAR,serum CRP level and QTcd in two groups(P=0.001 all).Compared with routine treatment group after treatment,there were significant reductions in LVEDd[(46.32±3.52)mm vs.(41.21±4.11)mm],LVESd[(40.68±4.42)mm vs.(35.18±4.16)mm],PAR [(7.56±1.25)% vs.(4.53±1.02)%],serum CRP level[(7.45±1.75)ng/L vs.(5.11±1.59)ng/L]and QTcd[(55.51±8.16)ms vs.(46.13±7.75)ms]in combined treatment group(P=0.001 all).Total effective rate of combined treatment group was significantly higher than that of routine treatment group(97.83%vs.82.61%,P=0.035).Conclusion:Compound Salvia dripping pill combined clopidogrel possesses significant therapeutic effect on CHF+VA.It can significantly improve cardiac function,relieve inflammatory response,inhibit platelet aggregation and improve VA symptom in these patients.
