慢性心力衰竭合并心房颤动的老年患者半乳糖凝集素-3水平及其临床价值

摘　　要：目的观察血清半乳糖凝集素-3(Gal-3)在慢性心力衰竭合并心房颤动的老年患者中的水平,探讨其临床价值。方法选取慢性心力衰竭合并心房颤动的老年患者60例(观察组)及同期就诊排除心力衰竭和心房颤动的老年患者30例(对照组),测定并比较两组血清Gal-3、N末端脑钠肽前体(NT-proBNP)、左心房内径(LAD)、左心室射血分数(LVEF)。随访18个月,记录两组患者心血管不良事件,分析血清Gal-3与NT-proBNP、LAD、LVEF及预后的关系。结果观察组患者血清Gal-3高于对照组[23.77(21.73,27.53)μg/L比8.02(6.65,10.28)μg/L,P<0.01];观察组患者血清Gal-3与NT-proBNP、LAD均呈正相关(r=0.387、0.876,均P<0.01),与LVEF呈负相关(r=-0.553,P<0.01);观察组患者心血管不良事件发生率随着血清Gal-3的上升而增加(P<0.01)。结论慢性心力衰竭合并心房颤动老年患者血清Gal-3升高与心功能恶化相关。Objective To explore the value of serum Galectin-3(Gal-3)in elderly patients with chronic heart failure(CHF)and atrial fibrillation(AF).Methods 60 senior patients with CHF and AF were enrolled as study group,30 elderly patients without heart failure and AF as control.Serum Gal-3,NT-proBNP,left atrial dimension(LAD),left ventricle eject fraction(LVEF)were measured.All patients were followed up for 18 months.Results Serum Gal-3 in the study group was significantly higher than that in the control group(P<0.01).In the study group,serum Gal-3 were positively correlated with serum NT-proBNP and LAD,and negatively correlated with LVEF(r=0.387、0.876、-0.553,all P<0.01).The incidence of cardiovascular adverse events in the study group increased with serum Gal-3(P<0.01).Conclusion Serum Gal-3 increase is related to deterioration of heart function in elderly patients with CHF and AF.
