探讨大黄不同炮制品与水蛭联合用药对大鼠糖尿病肾病肾脏的保护作用

摘　　要：目的观察大黄不同炮制品与水蛭联合用药对大鼠糖尿病肾病的肾脏保护作用方法通过注射浓度为55mg/kg的1%链腺佐菌素制备70只糖尿病肾病大鼠模型,分为正常组、模型组、替米沙坦对照组、生大黄+水蛭组、熟大黄+水蛭组、酒大黄+水蛭组、大黄炭+水蛭组,每组10只。每日灌胃给药1次,持续8周:在第2,4,6,8周末检测微量尿蛋白,将治疗前与治疗后向随机血糖进行对比,在末次给药后对肾重、AST、ALT、BUN、CRE进行检测结果糖尿病肾病大鼠经生大黄、熟大黄、酒大黄与水蛭合用干预后随机血糖、微量尿蛋白、肾重、AST、ALT、BUN、CRE均有所降低,大黄炭与水蛭合用后效果相对较差;HE染色发现各给药组与模型组比,发现糖尿病肾病大鼠肾小球缩小,系膜区变窄,肾小囊变宽,周围肾小管变性减轻,管腔变宽结论生大黄、熟大黄、酒大黃、大黄炭与水蛭合用可以延缓糖尿病肾病大鼠的病情发展从而起到对糖尿病肾病大鼠肾脏的保护作用,其中酒大黄效果较为明显,其机制可能与其活血化瘀作用较强有关,但仍需进一步研究。Objective To observe the renal protective effect of different processed Rhubarb products combined with hirudo on diabetic nephropathy in rats.Methods Seventy diabetic nephropathy rat models were established by injecting 1%streptozotocin at a concentration of 55 mg/kg.They were divided into normal group,model group,telmisartan control group,raw Rhubarb+hirudo group,cooked Rhubarb+hirudo group,wine Rhubarb+hirudo group,Rhubarb charcoal+hirudo group,10 rats in each group.The drug was administered orally once a day for 8 weeks.At the end of the 2nd,4th,6th and 8th week,microalbuminuria was detected.The blood sugar before and after treatment was compared.Renal weight,AST,ALT,BUN and CRE were detected after the last administration.Results After the intervention of raw Rhubarb,cooked Rhubarb,wine Rhubarb with hirudo,the random blood sugar,microproteinuria,kidney weight,AST,ALT,BUN and CRE of diabetic nephropathy rats decreased,but the effect of Rhubarb charcoal combined with leech was relatively poor.HE staining showed that the glomeruli of diabetic nephropathy rats decreased,mesangial area narrowed,renal capsule widened,and surrounding kidney was smaller than that of model group.The degeneration of the canal was alleviated and the cavity became wider.Conclusion The combination of raw Rhubarb,cooked Rhubarb,wine Rhubarb,Rhubarb charcoal and hirudo can delay the development of diabetic nephropathy rats and thus play a protective role on the kidney of diabetic nephropathy rats.The effect of Avine Rhubarb is more obvious,and its mechanism may be related to its strong effect of activating blood circulation and removing blood stasis,but further research is needed.
