四妙散加虫类药治疗湿热瘀互结型痛风性关节炎临床观察

摘　　要：目的观察四妙散加虫类药治疗急性痛风性关节炎的临床疗效。方法选择阳春市中医院2018年1月—2019年1月门诊及住院部治疗的湿热瘀互结型痛风性关节炎患者80例作为研究对象。采用随机数字表法将入组患者分为对照组与治疗组,每组各40例。治疗组给予四妙散加虫类药治疗,对照组口服布洛芬缓释片,2组患者均治疗2周。观察治疗前后2组患者血清VAS评分、UA、ESR及CRP浓度差异,并评估临床疗效及不良反应发生率。结果2组患者治疗前VAS评分、UA、ESR和CRP比较无统计学差异(P>0.05),治疗后治疗组的VAS评分比对照组VAS评分低,差异具有统计学意义(P<0.05),治疗后血清UA、ESR和CRP浓度比较,治疗组明显低于对照组,具有统计学差异(P<0.05),治疗组临床疗效由于对照组,差异具有统计学意义(P<0.05),治疗组不良反应发生率低于对照组,差异具有统计学意义(P<0.05)。结论针对湿热瘀互结型痛风性关节炎的患者,采用四妙散加虫类药治疗可明显减轻患者疼痛,抑制局部炎症反应,促进尿酸排泄,同时有较低的不良反应发生率,值得临床推广。Objective To observe the clinical effect of Simiao powder combined with worms Chinese medicine on acute gouty arthritis.Methods Clinical data of 80 patients who was treated in Yangchun hospital of traditional Chinese medicine from January 2018 to January 2019 with damp-heat and blood stasis type gouty arthritis were selected.Patients were divided into treatment group and control group by random number table method.There were 40 cases in each group.The treatment group was given Simiao powder combined with worm’s Chinese medicine treatment.The control group received Ibuprofen orally.Patients in both groups were treated for 2 week.The VAS score,serum UA,serum ESR and serum CRP concentrations of the two groups before and after treatment were observed.The clinical efficacy and incidence of adverse reactions were evaluated.Results There was no significant difference between the two groups in VAS score,UA,ESR and CRP before treatment(P>0.05).VAS score of the treatment group was lower than that of the control group after treatment,and the difference was statistically significant(P<0.05).After treatment,the concentrations of UA,ESR and CRP in the treatment group were significantly lower than those in the control group(P<0.05).The clinical efficacy of the treatment group was significantly different from that of the control group(P<0.05).The incidence of adverse reactions in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment with Simiao powder combined with worms Chinese medicine can significantly reduce the pain of patients with damp-heat and blood stasis type gouty arthritis,inhibits local inflammatory responses,promote the excretion of uric acid.At the same time,it has a low incidence of adverse reactions.It is worthy of clinical promotion.
