经外侧"L"型切口治疗跟骨骨折的疗效与体会

摘　　要：目的:探讨经外侧"L"型切口入路治疗跟骨骨折的疗效及降低术后切口并发症的体会.方法:回顾性分析66例(70足)跟骨骨折患者资料,早期采用传统外侧"L"型切口入路治疗44足,后期采用改良外侧"L"型切口治疗26足.观察术后切口愈合及评价功能恢复情况.结果:术后均获得随访,平均18个月(12-36个月).末次随访按Maryland足部功能评分系统进行疗效评定,优44足,良18足,可6足,差2足;Maryland足部评分平均为85.7分,整体优良率为88.6%.发生并发症17足,其中切口愈合不良5足,感染4足;8足给予换药或清创等后治愈,1例取出内固定接骨板后切口愈合;慢性疼痛8足.结论:经外侧"L"型切口入路治疗跟骨骨折,注重术中操作细节,可有效降低切口并发症的发生,获得满意的短期效果.Objective:To investigate the countermeasures and preventive measures for postoperative complications of calcaneal fractures with internal fixation via extensile L shape approach.Methods:Retrospective analysis of 66 cases(70 feet)with calcaneal fracture patient data,early adoption of the traditional lateral"L"shape approach incision in the treatment of 44 feet,and later improved lateral"L"shape approch incision for 26 feet.The postoperative wound complications and evaluation function were observed.Results:The average follow-up period of 66patients(70feet)was 18(12-36)months,Maryland Foot Score System was used to evaluate clinical funtional Results:Excellent in 44 feet,good in 18 feet,fair in 6 feet and poor in 2 feet.The average Maryland Foot Score was 85.7,and the excellent and good rate was 88.6%.There were 17 cases of complications,including 5 cases of poor wound healing,4 cases of wound infection and 8 cases of chronic pain.Conclusion:ORIF via the lateral"L"shape approach in the management of calcaneal fractures and attention to operation details,it can effectively reduce the occurrence of wound complications and obtain satisfactory short-term results.
