腰丛及坐骨神经联合阻滞在膝关节镜手术中的护理

摘　　要：目的:观察腰丛及坐骨神经联合阻滞在膝关节镜手术中的护理分析.方法:将2017年1月-2017年12月时期在我院骨科收治拟行腰丛及坐骨神经联合阻滞的膝关节手术患者总计107例,采取常规护理干预,设置为对照组.2018年1月我科开展针对性的护理干预,针对2018年1月-2018年12月时期收治另111例腰丛及坐骨神经联合阻滞的膝关节手术患者,设置为观察组.对照2组患者术后不同时间段肿胀、疼痛VAS评分,术后并发症发生率,患者对护理的满意度评价.结果:2组患者术后肿胀值于48小时达高峰随即下降,VAS值术后持续下降,在术后不同时间段观察组肿胀值、VAS评分明显低于对照组,观察组患者并发症发生率为0.9%,对照组并发症发生率为7.48%,观察组患者护理满意率为100%,对照组患者护理满意率为91.59%,差异具有统计学意义(P<0.05).结论:在腰丛及坐骨神经联合阻滞膝关节镜手术患者的护理中,采取针对性的护理,可明显减轻患者术后下肢肿胀与疼痛感,降低并发症概率,使患者更加满意,拉近护患距离,效果理想.Objective:To observe the nursing analysis of lumbar plexus and sciatic nerve block in knee arthroscopic surgery.Methods:A total of 107 knee surgery patients who were going to undergo combined lumbar plexus and sciatic nerve block in the orthopaedic department of our hospital in the near future(January 2017-December 2017)were treated with routine nursing intervention and set up as the control group.In January 2018,our department carried out targeted nursing intervention.Another 111 knee surgery patients with lumbar plexus and sciatic nerve block were selected as experimental group.The VAS scores of swelling and pain,the incidence of complications and the satisfaction of patients with nursing were compared between the two groups.Results:The swelling value of the two groups reached the peak at 48 hours after operation,and the VAS value continued to decrease.The swelling value and VAS score of the nursing group were significantly lower than those of the control group at different time after operation.The incidence of complications in the experimental group was 0.9%,the incidence of complications in the control group was 7.48%,the nursing satisfaction rate in the experimental group was 100%,and the nursing satisfaction rate in the control group was 91.59%.Academic significance(P<0.05).Conclusion:In the nursing of patients undergoing knee arthroscopic surgery with lumbar plexus and sciatic nerve block,targeted nursing can significantly reduce the swelling and pain of lower limbs,reduce the occurrence of postoperative complications,improve patients satisfaction with nursing,and shorten the distance between nurses and patients.
