摘 要:目的:研究机械性吸-呼气咳痰技术在机械通气患者气道清洁疗效和安全性。方法:本次选取研究对象为潮州市中心医院于2017年02月11日至2018年06月30日收治的60例机械通气患者,随机分为2组,对照组于常规方法治疗的基础上给予振动排痰仪、支气管镜吸痰处理;实验组于对照组治疗的基础上给予机械性吸-呼气咳痰仪辅助排痰,各30例。结果:实验组30例机械通气患者的抗生素应用时间、ICU入住天数及机械通气时间均显著短于对照组(P<0.05)。实验组30例机械通气患者的动脉氧分压、氧合指数显著高于对照组(P<0.05)。实验组30例机械通气患者的白细胞计数、C反应蛋白、血降钙素原明显低于对照组(P<0.05)。实验组30例机械通气患者的多器官功能衰竭发生率、并发症发生率及28天死亡率与对照组比较无显著性差异(P>0.05)。结论:机械性吸-呼气咳痰技术应用于机械通气患者中具有较高的应用价值,能够在保证安全性的基础上彻底清除肺内分泌物,值得应用及推广。Objective:To study the efficacy and safety of the mechanical suction-breath sputum technique in airway cleaning in patients with mechanical ventilation.Methods:The selection of the object of study of 60 patients with mechanical ventilation,02 Nov.2017 to 30 June 2018 were randomly divided into 2 groups,namely the control group on the basis of the conventional treatment for vibration expectoration drainage apparatus and Bronchoscopic sputum aspiration management;the experimental group on the basis of the conventional method to treat give mechanical suctionexhale,30 cases in each.Results:The experimental group of 30 patients with mechanical ventilation of antibiotic application time,mechanical ventilation and ICU stay days time is significantly shorter than those of the control group(P<0.05).Experimental group 30 cases of patients with mechanical ventilation of arterial oxygen partial pressure,oxygen index is significantly higher than those of the control group.Experimental group 30 cases of patients with mechanical ventilation white blood cell count,c-reactive protein and blood calcitonin original data is lower than the control group(P<0.05).Experimental group 30 cases of patients with mechanical ventilation on the incidence of multiple organ failure,the incidence of complications and 28 days mortality were no significant difference between the control group and the control group(P>0.05).Conclusion:Mechanical suction-expiratory technology used in mechanical ventilation in patients with sputum has high application value,to ensure safety on the basis of thoroughly remove secretions in the lung,is worthy of application and promotion.
