内镜辅助下穿皮内固定治疗髁状突颈下部骨折七例

摘　　要：目的总结内镜辅助下髁状突颈下部骨折口内入路穿皮内固定的治疗体会。方法回顾性分析延安大学附属医院口腔科2017年10月至2019年6月经过口内入路手术治疗的7例髁状突颈下部骨折患者的临床资料。总结手术方法、骨折内固定及术后伤口愈合等情况。结果 7例患者手术均顺利完成,骨折内固定稳定可靠,效果满意;穿皮操作未发现面神经损伤及涎腺损伤;6例伤口一期愈合,1例患者发生伤口积液感染,换药后好转,未影响骨折愈合;术后随访1~6个月,面部穿皮小切口疤痕不明显,患者咬合关系及张口度恢复满意。结论内镜辅助下经口内切口穿皮内固定治疗髁状突颈下部骨折,骨折固定可靠,术后并发症少,不影响面部皮肤美观。Objective To summarize the clinical application of endoscopy assisted percutaneous internal fixation in the treatment of condyle subcondylar fracture. Methods The clinical data of 7 cases of subcondylar fracture patients, who underwent endoscopy assisted treatment via intraoral incision in Department of Stomatology of the Affiliated Hospital of Yan’an University from October 2017 to June 2019, were respectively analyzed. The surgical methods, internal fixation of fractures and postoperative wound healing were summarized. Results All the 7 cases were successfully operated, and the internal fixation of fracture was stable and reliable with satisfactory effect. No facial nerve injury or salivary gland injury was found during skin penetration. Six cases had a primary wound healing, and one case had wound effusion infection, which was improved after dressing change and did not affect fracture healing. The patients were followed up ranged from one to six months. The occlusal relationship and mouth opening recovered satisfactorily, and the scar of small facial incision was not obvious. Conclusion For the treatment of condyle subcondylar fracture, endoscopy assisted percutaneous internal fixation has the advantages of reliable fracture fixation, fewer complications and no influence on facial beauty.
