利胆清胰汤治疗急性胆源性胰腺炎临床疗效的荟萃分析

摘　　要：目的运用荟萃分析的方法对口服利胆清胰汤治疗急性胆源性胰腺炎的临床疗效进行评价。方法计算机检索中国知网、万方数据、重庆维普、中国生物医学文献服务系统、中国科学引文索引数据库、PubMed以及Cochrane数据库,检索时限为建库至2019年2月。2名评价者独立筛选文献、提取资料和评价纳入研究偏倚后,采用RevMan 5.3软件进行荟萃分析。结果观察组患者临床有效率显著高于对照组(OR=4.04,95%CI:2.11~7.75,Z=4.21,P<0.0001);观察组患者中医证候总积分,急性生理与慢性健康评分,血、尿淀粉酶水平,白细胞计数,血清白介素-15、白介素-18水平均显著低于对照组(均P<0.05)。结论在常规治疗+内镜逆行胰胆管造影治疗的基础上联合利胆清胰汤治疗急性胆源性胰腺炎具有更好的疗效。Objective Use Meta-analysis to evaluate the clinical efficacy of Lidan Qingyi decoction in the treatment of acute biliary pancreatitis.Method The China National Knowledge Infrastructure,Wanfang Data,China Science and Technology Journal Database,China biomedical literature service system,Chinese Science Citation Database.PubMed and the Cochrane Library were retrieved from the time of database establishment to February 2019.Two reviewers screened the literatures,extracted data and evaluated the bias of the included trials independently.Afterwards,using RevMan 5.3 software,the meta-analysis was conducted.Result The clinical efficiency of Lidan Qingyi decoction combined with endoscopic retrograde cholangiopancreatography in the treatment of acute biliary pancreatitis(ABP)is higher than that in control group(OR=4.04,95%CI:2.11~7.75,Z=4.21,P<0.0001).The treatment reduced total score of TCM syndromes,APACHE-Ⅱ,amylase in urine and blood,serum interleukin-15 and interleukin-18 level in observation group were significantly lower than those in control group(all P<0.05).Conclusion On the basis of conventional treatment and endoscopic retrograde cholangiopancreatography combined with Lidan Qingyi decoction,oral treatment of biliary pancreatitis has a better effect.
