基于加速康复外科理念的护理策略在腹腔镜胰十二指肠切除术患者中的应用

摘　　要：目的:探讨基于加速康复外科(ERAS)理念的护理策略在腹腔镜胰十二指肠切除术(Lap PD)患者中的应用方法及效果。方法:将2015年1月1日~2016年3月31日23例Lap PD患者作为对照组,实施围术期常规护理策略;将2016年4月1日~2017年5月31日20例Lap PD患者作为观察组,实施基于ERAS理念的护理策略。比较两组护理效果。结果:与对照组相比,观察组术后开始下床活动时间、术后拔除胃管时间、术后拔除营养管时间、术后排气时间、开始进流质饮食时间提前(P<0.05),术后肠外营养使用时间、拔出引流管时间及术后住院时间缩短(P<0.05),护理满意度更高(P<0.05)。两组手术时间、术中出血量、术后并发症发生率、胰瘘发生情况等方面比较差异无统计学意义(P>0.05)。结论:基于ERAS理念的护理策略应用于Lap PD患者是安全、可行、有效的。Objective:To explore the application and effect of nursing strategies based on the concept of enhanced recovery after surgery(ERAS)in patients with laparoscopic pancreaticoduodenectomy(Lap PD).Methods:A total of 23 patients with Lap PD from January 1,2015 to March 31,2016 were taken as the control group and the perioperative routine nursing strategy was implemented.From April 1,2016 to May 31,2017,20 patients with Lap PD were selected as the observation group to implement the ERAS-based nursing strategy.The nursing effects of the two groups were compared.Results:Compared with control group,the ambulation time after operation,postoperative removal time of stomach tube,postoperative removal time of nutrition tube,postoperative exhaust time,and the starting time of liquid diet intake in the observation group were significantly shorter(P<0.05).The nursing satisfaction in the observation group was higher that of the control group(P<0.05).There was no significant difference between the two groups in operation time,intraoperative blood loss,postoperative complication rate and pancreatic fistula(P>0.05).Conclusion:The ERAS-based nursing strategy is safe,feasible and effective for Lap PD patients.
