摘　　要：目的评估顶体功能低下的药物治疗方案的应用效果。方法选择2013年5月至2015年5月北京大学深圳医院生殖医学中心诊治的顶体功能低下的78例男性患者为研究对象,其年龄范围23岁~49岁,不育时间范围2年~4年。采用改良kenedy法检测顶体酶活性。使用十一酸睾酮-他莫西芬-维生素A-卵磷脂联合药物治疗方案进行治疗,治疗时间1个月~4个月。结果与治疗前比较,治疗后患者精子密度增加(t=-5.05,P=0.000),前向运动精子比例(t=-2.14,P=0.036)其差异均具有统计学意义;精液体积(t=-1.93,P=0.057)、正常精子形态(t=0.24,P=0.811),差异无统计学意义。治疗前顶体酶活性为[40.3(31.1,44.2)]μIU/10~6精子,治疗后增加至[49.9(43.4,62.0)]μIU/10~6精子,其差异具有统计学意义(t=-8.18,P=0.00)。78例患者中,7例(9.0%)顶体活性下降,71例(91.0%)顶体活性上升,18例(23.1%)顶体活性未达到正常。治疗后,10例(12.8%)自然受孕,2例(2.6%)流产。结论十一酸睾酮-他莫西芬-维生素A-卵磷脂联合药物治疗方案可以有效改善顶体功能。Objective To assess the effectiveness of andriol, tamoxifen, vitamin A and lecithin on low acrosome. Methods 78 infertile males with low acrosome in our hospital from May 2013 to May 2015 were enrolled, aged ranging from 23 to 49 years old. Modified Kenedy test was used to test acrosome function. Andriol, tamoxifen, vitamin A and lecithin were used for 1 to 4 months. Results Sperm concentration(t=-5.05, P=0.000),progressive sperm(t=-2.14, P=0.036) raised while seminal volume(t=-1.93, P=0.057), normal morphology(t=0.24, P=0.811) did not show statistically significant difference after treatment. Acrosin was [40.3(31.1, 44.2)] μIU/10~6 sperm before treatment and raised to [49.9(43.4, 62.0)] μIU/10~6 sperm after treatment, with statistically significant difference(t=-8.18, P=0.00). In all the 78 males, the acrosin of 7 men(9.0%) decreased after treatment while the acrosin of 71 males(91.0%) raised and 10 couples(12.8%) got pregnant with 2 cases of miscarriage(2.6%). Conclusions The treatment of andriol, tamoxifen, vitamin A and lecithin is effective on low acrosin.
