摘 要:目的:探讨16层CT低管电压联合迭代重建技术在胸部检查中的应用价值。方法:选取120名胸部CT健康体检者,按照检查时管电压的不同将其分为120 kV组和80 kV组,每组60名。分别采用Karl 3DTM迭代算法和滤波反投影(FBP)算法对两组进行图像重建,两组分别获得Karl 3DTM迭代重建和FBP重建图像;比较两组图像的两种重建方法主动脉弓层面和气管分叉层面图像噪声、信噪比(SNR)及对比噪声比(CNR)差异,评价图像质量;记录并分析每次CT扫描的CT容积剂量指数(CTDIovl)和剂量-长度乘积(DLP),对比两组辐射剂量。结果:120 kV组Karl迭代重建与FBP重建图像和80 kV组Karl迭代重建图像与FBP重建图像的主动脉弓层面和气管分叉层面的图像噪声、SNR和CNR比较,差异均有显著的统计学意义(t80 kV组=15.778,t=15.953,t=10.974;t120 kV组=14.274,t=12.134,t=14.249;P<0.05)。两组图像质量评分具有高度一致性(Kappa系数≈0.71)。80 kV组的平均CTDIovl和DLP值比120 kV组分别下降70.54%和72.53%。结论:在16层胸部CT检查中,采用低管电压联合迭代重建技术能有效地降低辐射剂量,并可获得满足诊断要求的图像质量。Objective:To explore application value of 16-slices computed tomography(CT)low tube voltage combined with iterative reconstruction technique(IRT)in chest examination.Methods:120 health people who underwent physical examination with chest CT were selected and they were randomly divided into 120 kV group and 80 kV groups(60 cases in each group)according to the different tube voltage during the examination.Karl 3DTM iterative algorithm and filtered back-projection(FBP)algorithm were respectively used to reconstruct the images of the two groups,and corresponding reconstructed images of two groups were respectively obtained.The differences of image noise,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)at aortic arch level and tracheal bifurcation level of two reconstruction methods between two groups were compared.And the image qualities of two groups were further evaluated.The volumetric CT dose index(CTDIovl)and the dose-length product(DLP)of each CT scan were recorded and analyzed,and the radiation doses of two groups were compared.Results:The differences of image noise,SNR and CNR at aortic arch level and tracheal bifurcation level of Karl iterative reconstruction and FBP reconstruction between 120 kV group and the 80 kV group were significant(t80 kV group=15.778,t=15.953,t=10.974.t120 kV group=14.274,t=12.134,t=14.249,P<0.05).The scores of image quality of the two groups possessed higher consistency(Kappa coefficient≈0.71).Compared 120 kV group,the average values of CTDIovl and DLP of 80 kV group declined by 70.54%and 72.53%,respectively.Conclusion:In the 16-slices chest CT examination,low tube voltage combines with iterative reconstruction technique can effectively reduce the radiation dose and obtain image quality that can meet the diagnostic requirements.
