摘  要:目的:研究三维快速自旋回波(3D-SPACE)序列扫描对膝关节交叉韧带损伤的诊断价值。方法:选取医院收治的56例膝关节交叉韧带损伤患者,入院后均完成磁共振成像(MRI)常规序列扫描和3D-SPACE序列扫描,以关节镜检查为对照,比较3D-SPACE序列与常规序列诊断膝关节交叉韧带损伤的灵敏度、特异度和准确率,以及图像主观评分等级。结果:MRI常规序列诊断膝关节交叉韧带损伤灵敏度、特异度和准确率分别为87.80%、86.67%和85.96%;MRI 3D-SPACE序列诊断灵敏度、特异度和准确率分别为95.12%、93.33%和94.64%,3D-SPACE序列与关节镜检查吻合度(k=0.921,P<0.05)优于MRI常规序列(k=0.681,P<0.05);常规序列诊断完全撕裂、部分撕裂与关节镜符合度分别为85.71%(36/42)和92.86%(13/14),3D-SPACE序列诊断完全撕裂、部分撕裂与关节镜符合度分别为95.24%(40/42)和92.86%(13/14),两者比较差异无统计学意义。MRI两种序列诊断膝关节交叉韧带损伤图像主观评分等级比较差异无统计学意义(x^2=1.813,P>0.05)。结论:3D-SPACE对膝关节交叉韧带损伤软组织分辨率高,可清晰显示关节结构、韧带损伤情况,与关节镜诊断符合度高,诊断效能优于MRI常规序列。Objective:To study the diagnostic value of sequence scan of three-dimensional sampling perfection withapplication-optimized contrasts by using different flip angle evolutions(3D-SPACE)on cruciate ligament injury ofknee joint.Methods:56 patients with cruciate ligament injury of knee joint who received treatment in hospital wereselected,and they underwent routine sequence scan of magnetic resonance imaging(MRI)and 3D-SPACE sequencescan of MRI after admission.The arthroscopy examination was used as contrast method,and the sensitivity,specificityand accuracy,as well as subjective grading of the images of the diagnosis of cruciate ligament injury of knee joint of3D-SPACE sequence and routine sequence were compared.Results:The sensitivity,specificity and accuracy rate ofroutine sequence of MRI were 87.80%,86.67%and 85.96%respectively in diagnosing cruciate ligament injury of kneejoint.The diagnostic sensitivity,specificity and accuracy rate of 3D-SPACE sequence of MRI were 95.12%,93.33%and 94.64%respectively.And the conformity degree of 3D-SPACE sequence(k=0.921,P<0.05)and arthroscopyexamination was better than that of routine sequence of MRI and arthroscopy examination(k=0.681,P<0.05).Forcomplete desmorrhexis of joint and partial desmorrhexis of joint,the conformity degrees of routine sequence andarthroscopy were 85.71%(36/42)and 92.86%(13/14)respectively,and those of 3D-SPACE sequence of MRI andarthroscopy were 95.24%(40/42)and 92.86%(13/14)respectively,and the differences of that between two methodswere not significant.There was no statistically significant difference in the subjective score grade between 3D-SPACEsequence of MRI and routine sequence of MRI in diagnosing cruciate ligament injury of knee joint(x^2=1.813,P>0.05).Conclusion:3D-SPACE of MRI has high resolution on the soft tissue of cruciate ligament injury of knee joint,andit can clearly show joint structure and the condition of ligament injury,and it has high conformity degree with thediagnosis of arthroscopy.And the diagnostic efficacy of 3D-SPACE of M
