摘  要:目的:探讨生化分析仪及化学发光分析仪在手足口病(HFMD)患儿血清肌酸激酶同功酶(CK-MB)、肌钙蛋白(cTnI)及炎性细胞因子检测中的临床应用。方法:选择医院收治的80例HFMD患儿,参照医学临床诊断标准将其分为无合并心肌损害组(43例)及合并心肌损害组(37例),另将50名健康体检儿童纳入健康对照组,检测3组受检儿童血清CK-MB、cTnI及炎性细胞因子中细胞间粘附因子1(ICAM-1)、白细胞介素6(IL-6)和转化生长因子-β(TGF-β)的含量。结果:无合并心肌损害组及合并心肌损害组血清CK-MB和cTnI水平均显著高于健康对照组,差异有统计学意义(tCK-MB=7.790,t=13.448;tcTnI=13.833,t=17.749;P<0.05),而合并心肌损害组血清CK-MB和cTnI水平高于无合并心肌损害组,差异均具有统计学意义(t=6.884、t=6.843;P<0.05)。无合并心肌损害组及合并心肌损害组血清ICAM-1和IL-6水平均高于健康对照组,差异有统计学意义(tICAM-1=4.592,t=7.949;tIL-6=5.174,t=7.268;P<0.05),而合并心肌损害组血清ICAM-1和IL-6水平高于无合并心肌损害组,差异均具有统计学意义(t=5.193,t=4.423;P<0.05)。无合并心肌损害组及合并心肌损害组血清TGF-β水平均低于健康对照组,差异有统计学意义(t=3.903、t=5.931;P<0.05),而合并心肌损害组血清TGF-β水平低于无合并心肌损害组,差异有统计学意义(t=4.558,P<0.05)。在HFMD合并心肌损害组中,ICAM-1、IL-6与CK-MB均呈正相关关系(r=0.649,r=0.725;P<0.05)。结论:生化分析仪及化学发光分析仪对血清CK-MB、cTnI的检测,是HFMD患儿合并心肌损害的重要临床诊断手段,而炎性细胞因子ICAM-1、IL-6和TGF-β均参与了HFMD伴心肌损伤的免疫病理过程。Objective:To explore the clinical application of biochemical analyzer and chemiluminescent analyzer in the detections of serum CK-MB,cTnI and inflammatory cytokines of children with HFMD.Methods:80 children with HFMD who admitted in hospital were selected,and they were divided into no-combined cardiac damage group(group A,43 cases)and combined cardiac damage group(group B,37 cases)according to medically clinical standard.And 50 children who were health as the results of physical examination were divided into health control group(group C).And the contents of serum creatine kinase isoenzyme MB(CK-MB),cardiac troponin I(cTnI)and intercellular adhesion molecule-1(ICAM-1)in inflammatory cytokines,interleukin-6(IL-6)and transforming growth factor(TGF-β)of 3 groups were detected.Results:The results of serum CK-MB and cTnI of group A and B were significantly higher than those of group C(tCK-MB=7.790,t=13.448,tcTnI=13.833,t=17.749,P<0.05),respectively.And the results of serum CKMB and cTnI of group B were significantly higher than those of group A(t=6.884,t=6.843,P<0.05),respectively.The serum ICAM-1 and IL-6 levels of group A and group B were significantly higher than those of group C(tICAM-1=4.592,t=7.949,tIL-6=5.174,t=7.268,P<0.05),respectively.And serum ICAM-1 and IL-6 levels of group B were significantly higher than those of group A(t=5.193,t=4.423,P<0.05),respectively.The serum TGF-βlevel of group A and group B were significantly lower than those of group C(t=3.903,t=5.931,P<0.05),respectively.And serum TGF-βlevels of group B was significantly lower than that in group A(t=4.558,P<0.05).In group B,ICAM-1 and IL-6 appeared positive correlativity with CK-MB(r=0.649,r=0.725,P<0.05),respectively.Conclusion:The detections of biochemical analyzer and chemiluminescent analyzer on serum CK-MB and cTnI are importantly clinical diagnostic mean of children with HFMD that combines with cardiac damage.While inflammatory cytokines(ICAM-1,IL-6 and TGF-β)involved in the immunopathological process of HFMD with cardiac damage.
