丘脑出血破入脑室单双侧脑室外引流并发症的对比分析

摘　　要：目的探讨单、双侧脑室外引流术治疗丘脑出血破入脑室并发症的差异,为选择合适的治疗方法提供依据。方法回顾性分析2015年1月-2018年12月于本院神经外科采用脑室外引流术救治的继发于丘脑出血的中度脑室出血患者71例其中行单侧脑室外引流术的39例,行双侧脑室外引流术的32例,观察统计两组术后再出血、穿刺道出血、引流管异位、颅内感染的发生率及30 d病死率。结果双侧脑室外引流术的穿刺道出血发生率为34.38%,引流管异位的发生率为25.00%,较单侧高,差异具有统计学意义(P<0.05)。两组患者术后再出血、颅内感染的发生率及30 d病死率比较差异无统计学意义(P>0.05)。结论对于丘脑出血继发的中度脑室出血,单侧脑室外引流联合尿激酶灌注是安全有效的治疗方法。Objective To investigate the differences in the complications of unilateral and bilateral external ventricular drainage in the treatment of thalamic hemorrhage,so as to provide a basis for the selection of appropriate treatment. Methods 71 patients with moderate ventricular hemorrhage secondary to thalamic hemorrhage being treated byexternal ventricular drainage in our hospital from January 2015 to December 2018 were retrospectively analyzed. Among them, 39 patients were carried out unilateral external ventricular drainage, 32 cases were treated with doubled external ventricular drainage. The bleeding after surgery, bleeding at puncture, abnormal position of drainage tube, incidence of intracranial infection and 30-day mortality were observed. Results The incidence of bleeding of puncture in bilateral external ventricular drainage was 34.38%, and the incidence of abnormal position of drainage tube was 25.00%, which was higher than that of unilateral drainage, and the difference was statistically significant(P < 0.05). There was no significant difference in the incidence of postoperative recurrent hemorrhage, intracranial infection and 30-day mortality between the two groups(P > 0.05).Conclusion For moderate ventricular hemorrhage secondary to thalamic hemorrhage, unilateral ventricular external drainage combined with urokinase perfusion is a safe and effective treatment.
