双头加压空心螺钉内固定结合辨证论治治疗肩峰骨折临床疗效评价

摘　　要：目的:评估采用双头加压空心螺钉内固定治疗肩峰骨折的临床疗效。方法:回顾性分析应用双头加压空心螺钉内固定治疗的8例闭合性肩峰骨折患者,根据美国肩肘关节医师协会评分系统(ASES)进行功能评价。术后定期复查X线片,观察骨折固定及愈合情况。结合三期辨证论治,遣方用药。结果:8例患者均获得12个月有效随访,骨折处全部骨性愈合,愈合时间为2-4.5个月,平均3.2个月。末次随访时ASES得分平均90.4分,优良率为87.5%(7/8)。结论:采用双头加压空心螺钉内固定结合3期辨证论治可促进骨折愈合,手术时间短,肩关节功能恢复良好等优势,值得推广应用。Objective:The aim of this paper is to evaluate the outcome of surgical treatment withthe use of double head compression hollow screw for the acromial fracture.Methods eight cases of isolated acromial f ractures without associated injures were treated operatively with double head compression hollow screws.The American shoulder and elbow surgeons(ASES)were used to assess function of shoulder.The X-ray films were reviewed regularly to observe the fixation and healing of the fractures,and in combination with three-stage treatment based on syndrome differentiationof TCM.Results:Al 1 eight patients received effective follow-up.The time to bone union ranged from 2 to 4.5 months with an average of 3.2 months,and bony union in all 8 patients.There were no complications of loose and broken screws.At final follow-up,based on the ASES,excel lent to good results were obtained in 87.5%(7/8).Conclusion:Internal fixation of double head compression hollow screws is a kind of viable option for acromial fracture.It achieves bone union with few complications,less injures of soft tissues,fixed reliable,shorter time of operation,as well as favourable function of shoulder,be worthy of more popularization and application clinically,and accelerating ferentiation and treatment of traditional Chinese medicine(TCM).
