摘 要:目的探讨不同病理类型鼻息肉中血清淀粉样蛋白A(serum amyloid A,SAA)表达差异。方法招募依据EPOS2012指南诊断为慢性鼻-鼻窦炎伴鼻息肉(chronic rhinosinusitis with nasal polyps,CRSwNP)患者共26例,术中收集鼻息肉标本。采用荧光定量聚合酶链式反应检测鼻息肉中SAA表达;采用苏木精和伊红染色检测病理标本中嗜酸性粒细胞、中性粒细胞、浆细胞、淋巴细胞数量,并依据比值将CRSwNP分为嗜酸性粒细胞型(eosinophilic chronic rhinosinusitis with nasal polyps,ECRSwNP)与非嗜酸性粒细胞型(non-eosinophilic chronic rhinosinusitis with nasal polyps,non-CRSwNP);采用Spearman分析鼻息肉中SAA水平与临床病理资料的相关性。结果non-ECRSwNP中的鼻息肉中SAA水平显着高于ECRSwNP组(P<0.001)及健康对照组(P<0.001)。鼻息肉中SAA水平与嗜酸性粒细胞百分比呈现显著负相关而与中性粒细胞及淋巴细胞浸润呈现显著正相关。结论 SAA在不同病理类型的鼻息肉中表达具有差异。OBJECTIVE To evaluate the expression of serum amyloid A(SAA) in chronic rhinosinusitis with nasal polyps(CRSwNP) with different histological subtypes.METHODS A total of 26 adult CRSwNP patients were enrolled in this study.Nasal polyps were collected during the surgery.Quantitative real-time PCR assay was used to investigate the SAA mRNA level in polyp tissues.Hematoxylin and eosin(HE) staining was performed for histological assessment of CRSwNP.Based on the percentage of infiltrated eosinophils,CRSwNP was divided into eosinophilic CRSwNP(ECRSwNP) and non-eosinophilic CRSwNP(nonECRSwNP).Spearman rank analysis was used to measure the correlation between SAA level and the percentage of inflammatory cells.RESULTS The mRNA level of SAA in nonECRSwNP was significantly higher than in ECRSwNP(P<0.001) and in control samples(P<0.001).The SAA mRNA level was positively correlated with the percentage of tissue neutrophils and lymphocytes whereas negatively correlated with the percentage of tissue eosinophils.CONCLUSIONThe SAA level differs in nasal polyps with different endotypes.
