超分子水杨酸换肤术治疗玫瑰痤疮的疗效观察

摘　　要：目的观察30%超分子水杨酸换肤联合0.75%甲硝唑凝胶治疗丘疹脓疱型玫瑰痤疮(PPR)的临床疗效及安全性。方法PPR患者58例,随机分为两组,治疗组30例,对照组28例。治疗组采用30%的超分子水杨酸进行换肤治疗,每3周1次,共治疗3次。两组患者丘疹脓疱处早晚均外用0.75%甲硝唑凝胶,治疗期间均使用医用护肤品。治疗前后(第0,9周)由两位皮肤科医师于双盲下分别记数两组患者的面部丘疹和脓疱数,并采用玫瑰痤疮严重度4级评分法对红斑程度进行评分;同时患者对干燥程度进行自我评估。记录患者治疗期间(首次治疗至末次治疗结束后3周)的不良反应。结果两组患者治疗后面部丘疹、脓疱数目及红斑均较治疗前明显改善(均为P<0.001),且治疗组的改善程度较对照组明显(P<0.001)。治疗组的干燥程度于治疗前后无明显变化(P=0.09)。所有患者未见明显色素沉着、色素减退及瘢痕等不良反应。结论采用30%超分子水杨酸换肤联合0.75%甲硝唑凝胶治疗,可明显改善PPR患者面部的红斑症状,减少丘疹脓疱数,治疗效果好,患者耐受性好,安全性高。Objective To evaluate the efficacy and safety of combination therapy of 30%supramolecular salicylic acid peels and 0.75%topical metronidazole gel on papulopustular rosacea.Methods Fifty-eight patients with papulopustular rosacea were randomly divided into 2 groups,the treatment group(n=30)and the control group(n=28).The treatment group underwent 3 sessions of 30%supramolecular salicylic acid peels,each with a 3-week interval.Patients in both groups applied 0.75%topical metronidazole to the lesions twice daily.All patients applied medical skin care products twice daily during the treatments.At week 0 and 9,the numbers of papules and pustules were counted and the scores of erythema were assessed using the 4-point severity grading system by two dermatologists through blinded evaluation.Patients also evaluated their symptom of dryness at week 0 and 9.Adverse events during the treatments were documented(from the first treatment to 3 weeks after the last treatment).Results The numbers of papules and pustules at week 9 were significantly decreased from baseline at week 0,for both treatment and control groups(P<0.001 for both treatment and control groups).At week 9,this number for treatment group was also significantly reduced,compared with control group(P≤0.001).The scores of erythema at week 9 were observed with statistically significant improvements from baseline in both groups(P<0.001 for treatment group and P=0.001 for control group).In addition,the treatment group achieved significant progress in erythema score at week 9,compared with control group(P<0.001).The scores of dryness in treatment group had no significant change compared with baseline(P=0.09).No obvious hyperpigmentation,hypopigmentation or scarring was observed throughout the study.Conclusions The results of this study demonstrated that the combination therapy of 30%supramolecular salicylic acid peels and 0.75%topical metronidazole is safe,effective,and well tolerated in the management of papulopustular rosacea.It can improve the symptoms of erythema
