磁共振成像与多层螺旋CT诊断结肠癌致肠梗阻的临床价值分析

摘　　要：为探讨磁共振成像(MRI)与多层螺旋CT诊断结肠癌致肠梗阻的临床价值,选择2017年1月至2019年1月我院收治并经手术证实为结肠癌致肠梗阻的43例患者进行研究,患者术前均行MRI和多层螺旋CT检查,比较两种方法对结肠癌致肠梗阻的诊断效果及影像学特征。结果显示,MRI对结肠癌致肠梗阻的检出率(100%)明显高于多层螺旋CT(88.37%),漏诊率(0)明显低于多层螺旋CT(11.63%),P<0.05。MRI对狭窄肠段、侵犯浆膜下或肠旁组织、狭窄肠段与正常肠管交界处改变、缺血性结肠炎检出率分别为100%、97.67%、97.67%和39.53%,明显高于多层螺旋CT(90.70%、86.05%、83.72%和18.16%),差异均有统计学意义,P<0.05;MRI对盲肠扩张的检出率为79.07%,多层螺旋CT为76.74%,差异无统计学意义,P>0.05。结果表明,MRI对结肠癌致肠梗阻的诊断效果优于多层螺旋CT,临床应用价值更高。This study was to explore the clinical value of MRI and multi-slice spiral CT in diagnosing intestinal obstruction(IO)induced by colonic cancer(CC),enrolled 43 patients who were treated in author’s hospital(2017-01-2019-01)and surgically confirmed as IO from CC;before surgery inspected them by using MRI and multi-slice spiral CT;then compared diagnostic results and image features between both inspections.As results,in the detected rate,and in misdiagnostic rate,MRI was all significantly higher and lower than multi-slice spiral CT(100%vs 88.37%,0 vs 11.63%,all P<0.05);besides,the detected rate of MRI in stenosis intestinal segments,in invaded subserosa or the tissue near to intestine,in the change between normal intestinal segment and stenosis one,and in ischemic colitis were respectively 100%,97.67%,97.67%and 39.53%,all significantly higher than that of multi-slice spiral CT(i.e,90.7%,86.05%,83.72%and 18.16%),all P<0.05;what’s more,the detected rate of typhlectais had no statistical difference between both inspections(79.07%vs 76.74%,P>0.05).Results show that in diagnostic results,MRI is perfect to multi-slice spiral CT for IO from CC,therefore of higher clinical application value.
