解冻周期单囊胚移植与卵裂期双胚胎移植的比较:一项关于反复种植失败周期的研究

摘　　要：目的比较解冻周期中单囊胚移植(SBT)与卵裂期双胚胎移植(DET)的临床和实验室资料,探讨解冻周期SBT对反复种植失败患者助孕结局的影响。方法收集本院2016年1月至2017年12月因不明原因反复种植失败且剩余有冷冻储存的囊胚期或卵裂期胚胎的患者资料,并比较解冻周期109例SBT患者(SBT组)和336例DET患者(DET组)的临床条件及助孕结局(包括临床妊娠率、种植率和流产率)。结果相对于DET组,SBT组的临床妊娠率(54.1%vs. 47.6%)和种植率(56.0%vs. 30.1%)较高,流产率(2.8%vs. 7.1%)较低,但均无统计学差异(P>0.05),而SBT组多胎率(1.8%vs. 12.2%)显著低于DET组(P<0.05)。结论对部分原因不明反复种植失败的患者,解冻周期实施SBT可能会改善其助孕结局。Objective:To compare the clinical and laboratory data of single blastocyst transfer(SBT)and double cleavage-stag embryo transfer(DET)in frozen-thawed cycle,and explore the effect of SBT in frozen-thawed cycle on the pregnancy outcome of patients with repeated implantation failure.Methods:The clinical data of recurrent implantation failure patients for unexplained infertility and they had blastocyst or cleavage-stage embryos vitrified in our hospital from January 2016 to December 2017 were collected.The clinical outcomes including clinical pregnancy rate,implantation rate and miscarriage rate of 109 single blastocyst transfers(SBT group)and 336 double cleavage-stage embryo transfers(DET group)were compared.Results:Compared with DET group,the clinical pregnancy rate(54.1%vs.47.6%)and implantation rate(56.0%vs.30.1%)of SBT group were higher,while the abortion rate(2.8%vs.7.1%)was lower,but there was no statistical difference(P>0.05),while the multiple birth rate(1.8%vs.12.2%)of SBT group was significantly lower than that of DET group(P<0.05).Conclusions:Single blastocyst transfer may improve the pregnancy outcomes for some unexplained infertility and recurrent implantation failure patients.
