基于数据挖掘的老年类风湿关节炎患者血小板参数变化及关联规则分析

摘　　要：目的运用关联规则等统计学方法分析老年类风湿关节炎(RA)患者血小板参数的变化及其与其他实验室指标的相关性。方法收集并整理2012年6月至2016年12月在安徽省中医院风湿免疫科住院的确诊RA 4073例的病历资料,以年龄是否大于60岁分为老年组(≥60岁)和中青年组(<60岁),采用SPSS 21.0软件对患者血清血小板参数[血小板计数(PLT)、血小板分布宽度(PDW)、血小板平均体积(MPV)、血小板压积(PCT)、大型血小板比率(P-LCR)]、免疫指标[免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)、补体C3、补体C4]、炎性指标[类风湿因子(RF)、红细胞沉降率(ESR)、C反应蛋白(CRP)、白细胞计数(WBC)]、代谢指标[超氧化物歧化酶(SOD)、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A 1(ApoA 1)、载脂蛋白B(ApoB)]等实验室指标进行相关性分析,并通过SPSS Clementine 11.1软件Aprior模块分析血小板参数、凝血、免疫、炎症、代谢等实验室指标与年龄的关系。结果(1)1520例老年RA患者PLT值与正常参考值(125×109/L^350×109/L)相比较,正常的有1221例(80.33%),异常的有299例(19.67%),2553例中青年组患者PLT值正常的有2113例(82.77%),异常的有440例(17.23%)。(2)两组异常率相比较,老年组PDW、MPV、HDL-C降低较青年组明显(P<0.05或P<0.01),PCT、P-LCR、ESR、CRP、IgA、IgM、IgG、C3、C4、TG升高较青年组明显(P<0.05或P<0.01)。(3)PLT与CRP呈正相关(P<0.05或P<0.01),与其他指标及年龄无相关性(P>0.05);PDW与年龄、IgM、IgG、C3、TG、TC呈负相关(P<0.05或P<0.01),与其他所选实验室指标无相关性(P>0.05);MPV与年龄、IgM、IgG、C3、TG、TC呈负相关(P<0.05或P<0.01),与其他指标无相关性(P>0.05);PCT与年龄、IgM、IgG、C3、TG呈负相关(P<0.05或P<0.01),与其他指标无相关性(P>0.05)。(4)设定最小置信度为70%,最小支持度为20%。经Aprior模块分Objective To analyze the changes of peripheral blood platelet parameters in elderly patients with rheumatoid arthritis(RA)and its correlation with other laboratory parameters by statistical methods such as association rules.Methods The records of patients with confirmed RA who were admitted to the Department of Rheumatology and Immunology of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from June 2012 to December 2016 were collected and sorted into the elderly group(≥60 years old)and young and middle-aged patients(<60 years old).Peripheral blood platelet parameters(PLT,PDW,MPV,PCT,P-LCR),immune markers(IgA,IgM,IgG,complement C3 and complement C4),inflammatory markers rheumatoid factor(RF),Erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),white blood cell count(WBC),and the metabolic indexes of superoxide dismutase(SOD),triglyceride(TG),total cholesterol(TC),HDL-C,LDL-C,ApoA 1 and ApoB were analyzed by SPSS 21.0.And analysis of platelet parameters,coagulation,Immunology,inflammation,metabolism and other laboratory indicators and the relationship between age through the SPSS Clementine 11.1 software Aprior module.Results(1)The PLT value of 1520 elderly patients with RA was 1221(80.33%)normal and 299(19.67%)abnormal compared with the normal reference value(125×109/L-350×109/L)There were 2113 patients(82.77%)with normal PLT values in the youth group and 440 patients(17.23%)with abnormalities.(2)Compared with the young group,the PDW,MPV and HDL-C in the elderly group were significantly lower than those in the young group(P<0.05 or P<0.01).The PCT,P-LCR,ESR,CRP,IgA,IgM,C3,C4,TG increased significantly than the young group(P<0.05 or P<0.01).(3)There was no significant correlation between PLT and CRP(P<0.05 or P<0.01),and no correlation with other indexes and age(P>0.05).PDW was negatively correlated with age,IgM,IgG,(P<0.05 or P<0.01),but no correlation with other selected laboratory parameters(P>0.05).There was a negative correlation between MPV and age,IgM,IgG(P<0.05 o
