摘 要:目的调查乙型肝炎(乙肝)病毒携带者中,乙肝病毒复制水平与非酒精性脂肪肝(NAFLD)发病风险的相关性。方法纳入该院健康管理中心2017年8月至2018年11月进行体检的未接受过治疗的乙肝患者,分析其NAFLD的发病风险,并对其进行乙肝DNA检查,分析脂肪变性与乙肝病毒DNA相关性。结果最终共纳入345例患者,其中男204例,女141例。脂肪肝组患者的年龄、男性所占比例及体重指数均明显高于单纯慢性乙肝组,差异有统计学意义(P<0.05)。单因素分析发现,血糖、血压、总胆固醇、三酰甘油等代谢性因素均与NAFLD相关(P<0.05)。而多因素回归分析发现,性别、体重指数、降低的乙肝病毒DNA载量、血糖、三酰甘油、总胆固醇与NAFLD相关(P<0.05)。结论NAFLD可能对病毒复制存在潜在的负面影响。在慢性乙肝患者中,血糖、三酰甘油等代谢性因素与NAFLD的发生风险存在相关性。Objective To examine the association between hepatitis B virus(HBV)viral load and risk of nonalcoholic fatty liver disease(NAFLD).Methods The patients who were diagnosed with hepatitis B and not received prior treatment during health examination of health management center in the hospital from August 2017 to November 2018 were included.The risk of NAFLD was analyzed,and HBV DNA was examined,the correlation between steatosis and HBV DNA was analyzed.Results A total of 345 patients were included,204 patients were male,while 141 patients were female.The age,proportion of male,body mass index in patients of NAFLD were significant higher than those of simple chronic hepatitis B,the differences were statistically significant(P<0.05).Univariate analysis found that blood glucose,blood pressure,total cholesterol,triglyceride were associated with NAFLD(P<0.05).On multivariate analysis found that sex,body mass index,blood glucose,triglyceride,total cholesterol and depressed HBV DNA were associated with NAFLD(P<0.05).Conclusion NAFLD may be a potential negative effect on HBV replication.And blood glucose,triglyceride and other metabolic factors were correlated with NAFLD.
