“破、和、补”三期辨证用药理论在中医防治创伤性股骨头坏死中的应用

摘　　要：创伤性股骨头坏死是临床中常见的骨病之一。西医治疗创伤性股骨头坏死具有一定的效果,但存在手术创伤、长期预后欠佳等一系列问题。中医治疗疾病讲究辨证论治,针对疾病的不同临床表现予以分期辨证施治,效果满意。基于中医骨伤医学平乐正骨流派的"破、和、补"三期辨证用药治疗骨伤科疾病理论,科室结合临床将创伤性股骨头坏死分早、中、晚三期进行施治,早期以"破"为主,多用攻下逐瘀法、凉血祛瘀法、利水消瘀法、行气消瘀法;中期以"和"为主,多用通经活络法、调气活血法、活血生骨法;后期以"补"为主,多用气血双补法、补中益气法、补肾壮骨法。通过三期辨证用药,动态地防治创伤性股骨头坏死。Traumatic osteonecrosis of femoral head is one of the most common osteopathy in clinic.Western medicine treatment has a certain effect,but there are a series of problems,such as surgical trauma,poor long-term prognosis and so on.Traditional Chinese medicine pays attention to syndrome differentiation and treatment in treating diseases.According to different clinical manifestations and stages of diseases,the effect is satisfactory.Based on the theory of"Po,He and Bu"three phases of syndrome differentiation and medication in Pingle Orthopedic School of traditional Chinese orthopedic medicine,the Department divided the traumatic necrosis of femoral head into three phases:early,middle and late.The prescription was mainly used for"Po"in the early stage,and the syndrome differentiation was selected with the method of attacking the sputum,cooling the blood,reducing the phlegm,and elimination of phlegm;the middle of the prescription drug was based on"He",and most of the prescriptions were Tongjing active method and activating blood and qi method and Buxie Shenggu method.In the later period of the prescription,the drug was mainly used for"Bu",so Buzhong Yiqi method and kidney and strong bone method was used commonly.Through three stages of syndrome differentiation,dynamic prevention and treatment of traumatic necrosis of femoral head was achieved.
