耳内窥镜下鼓膜穿刺注药正负压置换治疗分泌性中耳炎的临床效果分析

摘　　要：目的研究耳内窥镜下鼓膜穿刺注药正负压置换治疗分泌性中耳炎的临床效果。方法随机抽取该院在2017年8月-2019年6月期间收治的80例分泌性中耳炎为研究对象,按照治疗方案划分为对照组(n=40)与观察组(n=40),对照组应用保守治疗方案,观察组应用耳内窥镜下鼓膜穿刺注药正负压置换治疗方案,对两组患者治疗效果、听力恢复时间、咽喉管功能评分、不良反应、随访半年后听阀、静态声顺值、耳峰压值及复发率进行评价与分析。结果观察组治疗总有效率(97.50%)高于对照组(77.50%)(χ~2=14.875,P<0.05);观察组听力恢复时间(20.25±3.02)h低于对照组(28.45±3.81)h,而咽喉管功能评分(8.95±1.23)分高于对照组(6.15±1.47)分(t=13.478、8.205,P<0.05);观察组不良反应(12.50%)与复发率(5.00%)均低于对照组(32.50%、22.50%)(χ~2=11.469、12.911,P<0.05);观察组随访半年后听阀(28.12±3.57)dBnHL、静态声顺值(-24.69±2.23)mmH2O及耳峰压值(0.96±0.05)m L均优于对照组(42.25±3.85)dBnHL、(-39.25±3.26)mmH2O、(0.53±0.02)mL(t=23.054、13.258、10.662,P<0.05)。结论对于分泌性中耳炎采取耳内窥镜下鼓膜穿刺注药正负压置换治疗效果尤为显著,能够有效缓解患者的临床症状,改善听力和咽喉管功能,减少不良反应及复发情况,有助于提升患者的生活质量,在临床上具有较高的应用价值。Objective To study the clinical effect of positive and negative pressure replacement of tympanic membrane puncture injection under otoscope for the treatment of secretory otitis media.Methods A total of 80 patients with secretory otitis media treated in the hospital from August 2017 to June 2019 were randomly selected as study subjects,and were divided into the control group(n=40)and observation group(n=40)according to the treatment plan,and the control group with conservative treatment,the observation group was treated with positive and negative pressure replacement of eardrum with tympanic puncture injection.The treatment effect,hearing recovery time,throat function score,adverse reactions,listening valve,static status after six months of follow-up conformity value,ear pressure,and recurrence rate were evaluated and analyzed.Results The total effective rate of treatment in the observation group(97.50%)was higher than that in the control group(77.50%)(χ~2=14.875,P<0.05).The hearing recovery time(20.25±3.02)h in the observation group was lower than that in the control group(28.45±3.81)h.While the throat function score(8.95±1.23)points was higher than the control group(6.15±1.47)points(t=13.478,8.205,P<0.05);the adverse reactions(12.50%)and recurrence rate(5.00%)in the observation group were lower than the control group(32.50%,22.50%)(χ~2=11.469,12.911,P<0.05);the observation group was followed by listening valve(28.12±3.57)dBnHL,static sounding value(-24.69±2.23)mmH2 O,and peak ear pressure after six months of follow-up value(0.96±0.05)mL was better than the control group(42.25±3.85)dBnHL,(-39.25±3.26)mmH2 O,(0.53±0.02)mL(t=23.054,13.258,10.662,P<0.05).Conclusion For otitis media with secretory otitis media,the effect of positive and negative pressure replacement of eardrum tympanic puncture injection is particularly significant,which can effectively alleviate the clinical symptoms of patients,improve hearing and throat functions,reduce adverse reactions and recurrence,and help improve the quality of
