摘　　要：目的分析微创手术入路治疗肱骨外科颈骨折的临床疗效。方法该次将该院于2016年3月-2019年3月收治的60例肱骨外科颈骨折患者作为研究对象,按随机分组法分成两组,各30例;对照组采用三角肌胸大肌间隙常规手术入路方法治疗,观察组采用闭合复位经皮微创锁定钛板内固定术治疗,进一步比较两组手术治疗效果。结果两组手术时间差异无统计学意义(t=1.283,P>0.05),观察组术中出血量明显少于对照组,切口长度明显短于对照组,愈合时间明显短于对照组,差异有统计学意义(t=6.776、8.273、6.289,P<0.05);观察组Neer评分明显高于对照组,VAS评分则明显低于对照组,两组数据差异有统计学意义(t=8.273、5.672,P<0.05);观察组肩关节康复优良率为96.67%,明显高于对照组的80.00%,两组数据差异有统计学意义(χ^2=8.753,P<0.05)。结论微创手术入路治疗肱骨外科颈骨折疗效显著,可降低术中出血量,减少软组织剥离,避免肱骨头血供破坏,及早缓解疼痛,外形美观的同时,改善患者肩关节功能,手术安全可靠。因此,值得采纳及应用。Objective To analyze the clinical effect of minimally invasive surgical approach in the treatment of humeral surgical neck fractures.Methods This time our hospital will be from March 2016 to March 2019,60 patients with humeral surgical neck fractures were treated as research objects and divided into two groups by random grouping,30 cases each.The control group was treated with conventional surgical approach of deltoid pectoralis major muscle space.The observation group was treated with closed reduction and percutaneous minimally invasive locking titanium plate internal fixation,and the surgical treatment effects of the two groups were further compared.Results There was no significant difference in the operation time between the two groups(t=1.283,P>0.05).The amount of bleeding in the observation group was significantly less than that in the control group,the length of the incision was significantly shorter than that of the control group,and the healing time was significantly shorter than that of the control group(t=6.776,8.273,6.289,P<0.05);the Neer score of the observation group was significantly higher than that of the control group,and the VAS score was significantly lower than that of the control group.The data difference between the two groups was statistically significant(t=8.273,5.672,P<0.05);the excellent and good recovery rate of the shoulder joint in the observation group was 96.67%,which was significantly higher than the 80.00%in the control group.The data difference between the two groups was statistically significant(χ^2=8.753,P<0.05).Conclusion The minimally invasive surgical approach is effective in treating humeral surgical neck fractures,which can reduce intraoperative blood loss,reduce soft tissue stripping,avoid disruption of blood supply to the humeral head,relieve pain early,and improve the shoulder joint function while the appearance is beautiful.The operation is safe and reliable.Therefore,it is worth adopting and applying.
