摘 要:目的探讨老年心力衰竭患者给予心肌能量代谢药物治疗效果。方法方便选取2017年5月—2018年4月该院诊治老年慢性心力衰竭患者84例进行研究,用随机分组表法将患者分为常规组(强心、利尿、扩血管等常规治疗,42例)和干预组(常规治疗联合曲美他嗪治疗,42例),比较两组患者治疗前后心肌能量消耗(MEE)、心室结构及心脏功能。结果治疗12个月,两组患者MEE、LVD、LVS水平显著降低,但干预组患者水平[(81.6±10.4)Cal/min、(42.7±4.6)mm、(40.1±3.5)mm]下降程度更显著,差异有统计学意义(t=10.658、10.283、7.380,P<0.05)。治疗12个月,常规组与干预组BNP水平为(236.4±25.8)pg/mL、(144.7±19.8)pg/mL显著下降;LVEF、6MWT为(46.1±2.8)%、(306.4±38.1)m;(52.3±4.1)%、(337.9±36.5)m,水平显著升高,但干预组患者改善程度更显著,差异有统计学意义(t=18.273、8.093、3.869,P<0.05)。结论心肌能量代谢药物在老年心力衰竭患者中治疗,能显著降低患者心肌耗氧量、改善心室结构,提高心脏功能。Objective To investigate the effect of myocardial energy metabolism drugs in elderly patients with heart failure.Methods 84 patients with chronic heart failure in the hospital from May 2017 to April 2018 were convenient selected and enrolled in this study.The patients were divided into routine group by randomized group method(conventional therapy such as cardiac,diuretic and vasodilatation,42 cases)and the intervention group(conventional treatment combined with trimetazidine,42 cases),compared myocardial energy expenditure(MEE),ventricular structure and cardiac function before and after treatment.Results After 12 months of treatment,the levels of MEE,LVD and LVS were significantly lower in the two groups,but the levels of the intervention group[(81.6±10.4)Cal/min,(42.7±4.6)mm,(40.1±3.5)mm]the difference was statistically significant(t=10.658,10.283,7.380,P<0.05).At 12 months of treatment,the BNP levels in the conventional group and the intervention group were(236.4±25.8)pg/mL,(144.7±19.8)pg/mL;LVEF and 6MWT were(46.1±2.8)%,(306.4±38.1)m;(52.3±4.1)%,(337.9±36.5)m,the level was significantly increased,but the improvement of patients in the intervention group the difference was statistically significant(t=18.273,8.093,3.869,P<0.05).Conclusion Myocardial energy metabolism drugs in elderly patients with heart failure can significantly reduce myocardial oxygen consumption,improve ventricular structure and improve cardiac function.
