不同温度冲洗液在肝胆管结石患者术后纤维胆道镜检查中的应用效果

摘　　要：目的探讨不同冲洗液温度对纤维胆道镜取石术后患者生命体征和并发症的影响。方法选取2016年5月至2018年2月我院进行术后纤维胆道镜检查的600例患者作为研究对象,将患者随机等分为A,B,C 3组,其中A组患者给予温度为20~25℃的冲洗液,B组给予温度26~30℃的冲洗液,C组给予温度为31~35℃恒温冲洗液。比较3组患者冲洗前后生命体征变化以及术后并发症发生情况。结果A,B组患者冲洗后体温、心率、呼吸频率均明显高于冲洗前(P<0.05),但冲洗前后收缩压、舒张压比较差异无统计学意义(P>0.05);C组患者冲洗前后生命体征变化比较差异无统计学意义(P>0.05)。C组患者冲洗后发热、寒颤、胃肠道反应等并发症发生率明显低于A,B组(P<0.05),但3组患者胆道出血、胆瘘发生率比较差异无统计学意义(P>0.05)。结论术后纤维胆道镜检查,采用恒温冲洗液(31~35℃)能够有效维持患者生命体征,降低术后并发症的发生率,减轻患者不适,有效地促进患者恢复,其操作简单方便,值得推广应用。Objective To investigate the effects of different temperature flushing fluid for vital signs and complications of patients undergoing choledocholithotomy.Methods We selected 600 cases of patients who underwent postoperative fiber choledochoscopy in our hospital from May 2016 to February 2018 as the study subjects.The patients were randomly divided into three groups:A,B,and C.Among them,the temperature in group A was given by flushing fluid at 20~25℃,group B was given a flushing fluid with a temperature of 26~30℃,group C was given a constant temperature flushing fluid with a temperature of 31~35℃.The changes of vital signs and postoperative complications were compared between the patients of the three groups.Results After washing,the body temperature,heart rate,and respiratory rate of patients in group A and B were significantly higher than those before washing(P<0.05),but there was no statistically significant difference in systolic pressure and diastolic blood pressure before and after washing(P>0.05).There was no significant difference in the vital signs of patients in group C before and after washing(P>0.05).The incidence of complications such as fever,chills,and gastrointestinal reactions after washing in group C was significantly lower than those in groups A and B(P<0.05),but there was no significant difference in the incidence of hemobilia and bile fistula in the three groups(P>0.05).Conclusion In postoperative fiber choledochoscopy,the use of constant temperature flushing fluid(31~35℃)could effectively maintain the vital signs of patients,reduce the incidence of postoperative complications,reduce patient's discomfort,effectively promote patient's recovery,and its operation was simple and convenient,which was worth popularizing and applying.
