制约社区预约转诊制度实施因素研究

摘　　要：目的:探讨社区预约转诊中存在的矛盾核心问题及制约预约转诊实施的因素。方法:2017年2月-2018年2月选取社区卫生服务中心15家和二级医院3家的预约转诊情况,对有转诊需求的3762例患者分析制约社区预约转诊制度实施因素。结果:1年来社区需要转到二级医院就诊的患者3762例,实际预约转诊数为832例,转诊就诊成功821例,其中需要预约转诊与实际预约转诊的比例为4.52:1,转诊成功率为98.7%,转诊成功患者满意度为50%。较多人认为制约预约转诊因素包括医疗信息交流不畅、预约转诊制度知晓率低、无预约转诊流程、接受转诊医院未开通绿色通道、无激励及引导机制,社区医生未主动预约转诊等。结论:延庆地区虽然实行了预约转诊,但实际预约转诊成功率及转诊满意度不高。Objective:To discuss the contradictory core problems in community appointment referral and the factors that restrict the implementation of appointment referral.Methods:The appointment and referral status of 15 community health service centers and 3 secondary hospitals were selected from February 2017 to February 2018.We analyzed the factors restricting the implementation of community appointment referral system for 3762 patients with referral needs.Results:In the past year,there were 3762 patients in the community who needed to be transferred to secondary hospitals.The actual number of appointments for referrals was 832,and 821 cases were successfully referred,among them,the ratio of appointment referral to actual appointment referral was 4.52:1,the referral success rate was 98.7%,and the satisfaction rate of successful referral patients was 50%.Many people thought that the factors that restrict appointment and referral include poor communication of medical information,low awareness of the appointment and referral system,no appointment referral process,no green channel in the referral hospital,no incentive and guidance mechanism,and community doctors not actively making appointment referrals and so on.Conclusion:Although appointment referrals have been implemented in Yanqing,the actual success rate and satisfaction with referrals are not high.
