摘 要:目的分析贫血与钙、铁、锌、铜、镁等微量元素缺乏和超标的关系,探讨贫血对神经心理发育的影响。方法选取2016年10月1日至2017年9月30日在兰州大学第一医院儿童保健科健康体检的0~1岁婴儿555例,检测其末梢血常规以及微量元素钙、铁、锌、铜及镁含量,并对其中259例婴儿进行Gesell发育评估,对数据的不同参数进行统计学分析。结果①兰州城区0~1岁婴儿贫血患病率为21.44%(119/555),以轻度贫血为主,以小细胞低色素性贫血为主,男女婴贫血患病率比较差异无统计学意义(P>0.05)。②微量元素缺乏率由高到低依次为铁15.31%(85/555)、钙11.89%(66/555)、锌8.46%(47/555),铜和镁元素缺乏未检出。③微量元素超标率由高到低依次为铁9.18%(51/555)、锌7.56%(42/555),钙、铜及镁元素超标未检出。④贫血组婴儿微量元素铁、锌含量明显低于非贫血组(P<0.05),两组间钙、铜及镁含量比较差异无统计学意义(P>0.05)。⑤贫血组婴儿大动作、精细动作评分显著低于对照组(P<0.05),两组适应行为、语言行为和个人-社交评分比较差异无统计学意义(P>0.05)。结论兰州城区0~1岁婴儿贫血患病率较高,存在微量元素铁、钙和锌缺乏,亦存在铁和锌元素超标。贫血与铁、锌缺乏存在相关性,且影响婴儿精细动作与大动作发育。Objective To study the relationship between anemia and levels of trace elements of calcium,iron,zinc,copper and magnesium,and explore the effect of anemia on neuropsychological development.Methods A total of 555 infants from 0 to 1 year old,who were examined in the Department of Child Health in the First Hospital of Lanzhou University from Oct.1st,2016 to Sep.30th,2017,were included for peripheral blood tests and trace element calcium,iron,zinc,copper and magnesium content.Among them,259 infants underwent Gesell developmental assessment,and the data were analyzed statistically.Results①The prevalence of anemia in infants aged 0-1 year old in Lanzhou was 21.44%(119/555),the dominant type was mild anemia,the blood cell morphology was small cell hypochromic anemia.There was no significant difference in the prevalence of anemia between male and female infants(P>0.05).②Trace element deficiency rate from high to low:iron 15.31%(85/555),calcium 11.89%(66/555),zinc 8.46%(47/555),copper and magnesium deficiency were not detected.③Trace element exceeding rate from high to low:iron 9.18%(51/555),zinc 7.56%(42/555),calcium,copper and magnesium excessiveness were not detected.④The content of trace elements of iron and zinc in the anemia group was significantly lower than that in the control group(P<0.05).There was no significant difference in calcium,copper and magnesium between the two groups(P>0.05).⑤The scores of major motion and fine motion in the anemia group were significantly lower than those in the control group(P<0.05).There were no statistically significant differences in adaptive behavior,language ability and personal-social behavior score between the two groups(P>0.05).Conclusion The prevalence of anemia in infants aged 0-1 year old in Lanzhou is high,and there are trace elements such as iron,calcium and zinc deficiency,while iron and zinc excessiveness also exist.There is a correlation between anemia and iron and zinc deficiency,which affects the development of fine motion and major motion of infants.
